Celebrated a very very special graduation yesterday that was 3 YEARS in the making, after 3 IVF cycles and 3 FETs, finally we reached this special milestone 🥹 So so so grateful for this couple’s enduring trust, perseverance and positive attitude through it all — they showed up each and every time, ready to take the next step no matter how difficult. This little one is so so loved already ✨ Tears may have been shed, but so so overjoyed to be “releasing” this special couple to the OBGYN!!! Congratulations!!!!

Congratulations to Dieudonne, NSUMDCOM Class of 2027 on the completion of her REI Rotation 🙌🏻 As a future OBGYN, I know her patients will be in good hands!!! For her end of rotation project, Dieudonne designed this excellent patient educational handout on Chronic Endometritis — a complicated and sometimes controversial topic yet very common in our fertility patients!! I cannot wait to share this resource with patients 🙌🏻 Well done on a wonderful rotation, and best of luck in your future career!!!

✨Welcome to the World to this International Miracle!!!! So elated to here of this little one’s safe arrival after nearly 4 years of treatments ✨
This couple travelled from Panama to seek care after 2 miscarriages, 3 IVF cycles and 3 unsuccessful FET treatments. We dove deep into all factors that may have contributed to their treatment failures and uncovered significant DNA Fragmentation in addition to numerous immunological factors. We undertook a new IVF cycle with specific egg and s***m optimization techniques and generated FIVE beautiful supplied embryos 🙌🏻 Next, we designed a transfer treatment to address each and every immunological finding (with the careful monitoring in collaboration with the guru of course!!), all coordinated internationally from the couple’s home in Panama. We were ecstatic to learn of our positive BhCG after transfer with excellent rise — already further than the couple had previously achieved in their past biochemical miscarriages 🙏🏻 With each and every milestone, we held our breaths, but each and every milestone brought new joy and happiness. Finally, the couple can now share their miracle triple rainbow baby has arrived safely and can be held in their arms 🫶🏻 What an honor to have been able to participate in your care — thank you for your trust, perseverance, and unwavering optimism in the face of your difficult journey and CONGRATULATIONS on your new arrival 🩵

✨Welcome to the World to this Absolute MIRACLE, who was nearly overlooked as Aneuploid Embryo generated years earlier 🤯
This couple had celebrated the birth of their first child a year prior — his story was a miracle in and of itself! The couple had suffered severe male factor infertility and decided to foster parent and then adopt their first child. Revisiting science, they pursued a “Hail Mary” IVF treatment resulting in just two embryos — one genetically tested but found to be aneuploid, the other not strong enough to be tested. Their first miracle biologic son was the result of the transfer of that “weaker” second embryo — now a perfectly beautiful an healthy little boy 🥹
A year after their birth, the couple requested an appointment to discuss the transfer of their aneuploid embryo. This is not traditionally done by any reputable US Fertility Center. However, just in case, I accessed our old medical records system to review the embryo genetics — much to ALL of our surprise, I shared with the couple that their embryo was in fact a Segmental Aneuploid, and we now in fact do have a limited amount of data to suggest these embryos may result in healthy live birth, though at a lower rate than euploid embryos. Segmental aneuploid embryos have small pieces of chromosomes missing or duplicated, rather than an entire chromosome. Because the change is only in a segment, some of these embryos may still have the potential to develop into healthy pregnancies 🤯 The couple was ecstatic. Following approval by our physicians as a “protocol breach” and extensive genetics counseling, we pursued the couple’s second FET. With zero expectations but tremendous faith, the couple met and surpassed each milestone, including 2nd trimester amniocentesis and extensive monitoring throughout their pregnancy. I am elated to learn of the safe arrival of this SECOND absolute miracle for this special couple ✨

HUGE CONGRATULATIONS to this International Couple who are FINALLY holding their miracle baby after experiencing 🙌🏻🙌🏻🙌🏻
We first met in June 2024 — they travelled to Miami from Mexico for our consultation 🤯 Though this couple was young, reviewing their extensive fertility history before our meeting, I knew this would not be a straightforward case. The couple had already completed 2 IVF rounds yielding just two euploid embryos, and unfortunately both FETs failed to implant. We faced endometriosis, poor response to stimulation in spite of adequate AMH, and recurrent implantation failure. I recommended a new stimulation with protocols to attempt to improve embryo quality as well as additional testing to evaluate the failed implantations. We generated two euploid embryos and proceeded to FET with confidence. Unfortunately, after everything, we experienced failed implantation for their 3rd euploid embryo 🥺 Devastated, yet with prevailing strength to continue, we met and reexamined everything. After a few FET protocol tweaks, we proceeded to FET again and decided to transfer our last Day 7 euploid embryo with another untested embryo. 10 days later, the couple received the best news possible in their last 5 years of TTC — they are PREGNANT!!! The beta rose, we had 1st OB ultrasound and discovered one of the two embryos implanted 😅 The pregnancy progressed, and prospered, and I am SO thrilled to hear of the news of this miracle baby’s arrival 🙌🏻🙌🏻🙌🏻 To this day, we will never know if it was the tested or untested embryo that stuck, as our euploid embryo was a BOY 💙 and the untested was unknown ✨

Congratulations to Chaveli, NSUMDCOM MS4, on the completion of her REI Elective rotation! She plans to pursue a career in Anesthesiology (don’t worry, I did my best to redirect her career plan 🤣), but after her 3rd year single day experience at IVFMD, she knew she wanted to dive further into reproductive medicine 🙌🏻
For her end of rotation project, Chaveli designed a patient educational handout on the Thyroid as it relates to fertility — a subject we address for every single patient pursuing fertility treatment and pregnancy! Thank you for a wonderful rotation, and best of luck in your future — your patients will be fortunate to be in your care 🫶🏻

✨Welcome to the World to this Little Miracle — our “last chance” embryo✨
After four devastating miscarriages, including the loss of a euploid embryo, we undertook numerous tests and identified a few possible causes. However, our biggest barrier remained the uterus — distorted and enlarged by adenomyosis, this was a barrier we could not “fix” through surgery or medication. Adenomyosis is “endometriosis of the uterus” and causes thickened wall of the uterus. In this patient’s case, the uterus was SO large that on one of her ultrasounds, we could only view the gestational sac abdominally (when usually a vaginal ultrasound gives best view) — the top of the uterus was THAT high 🫣 Given this barrier which we would never be able to completely “cure” I discussed with the couple a recommendation for gestational carrier after our euploid embryo miscarriage. The couple was obviously broken and devastated — after so much loss and heartbreak, the thought of gestational carrier was too much to bear. They pleaded for one more chance, with the agreement that if unsuccessful, our LAST euploid embryo would be saved for a carrier.
Cautiously, but with SO much hope & prayer, and a new FET regimen, we proceeded with transfer. With each milestone, we held our breaths — couples experiencing miscarriage never completely celebrate until we pass prior times of devastation, and even then, they often are afraid to let down their guard, scarred by their past trauma. Well sure enough, this couple steadily passed each milestone, step by step, graduated to the OBGYN, and made it to NEAR full term. I was SO elated to hear of their miracle baby’s safe arrival, FINALLY after so many years of trying 🫶🏻
Congratulations to this incredible couple, what an incredible end to this long long journey ✨ Thank you for your enduring trust and unstoppable perseverance ✨

Welcome to the World (FINALLY!!!!) to this little miracle Triple Rainbow, and the FIFTH in his family lineage with his namesake 🥹
This couple’s journey began FIVE years ago this month. We faced severe s***m factor and known autoimmune diagnoses. But nothing has prepared us all for the long journey ahead. Over the ensuing FIVE years, the couple underwent FIVE IVF cycles, including a cycle out of state with one of the premier IVF center in the country (unsuccessfully 🥺). Our FET seemed to be going so perfectly, until a devastating 9week miscarriage while traveling internationally 😭. Finally on our 3rd FET, this little man stuck and stayed, with a combination of numerous immunologic treatments and anticoagulation to ensure we would not face miscarriage again. After 5 years and 5 IVF cycles, it is a perfect fit for this little boy who is also the 5th in his family with his name 🥰 Destined to be his magic number for life 🫶🏻
Through the ups and many downs of their journey, this couple stayed steadfast and focused on their ultimate goal and never gave up. Thank you for your enduring trust, for returning to care after an unsuccessful 2nd opinion elsewhere, and for giving this treatment every ounce of effort and positivity you could muster — YOU DID IT!!!

P.S. unfortunately little Tommy slipped and fell into dog 💩 minute 2 of this photoshoot and we were unable to fully emerge him from his disappointment (literally smeared down his entire back and side, but magically hidden by the excellent camera angles!)….life with boys 🫠

🌈 Congratulations to this incredible couple on FINALLY holding their miracle rainbow baby in their arms🌈
Most, if not every couple, who has experienced miscarriage and especially recurrent miscarriage are traumatized and highly anxious in their subsequent pregnancies — feeling on edge until they “pass that point” where they’ve had loss and disappointment. Many don’t truly breathe relief until holding their baby in their arms. This couple is no exception. After welcoming their son through a fairly straightforward IVF treatment 5 years ago, the couple (and I!) expected similar success as we pursued FET for baby #2. Baby #1 was a fresh ET but two embryos remained cryopreserved (untested). Unfortunately, neither embryo implanted, which I counseled is MOST likely due to abnormal embryo genetics. The couple took a break and tried naturally but suffered TWO subsequent miscarriages despite their young age 😞 We subsequently undertook a more in depth miscarriage analysis and uncovered a few possible explanations. Equipped with this information we proceeded with new IVF cycle with genetic testing this time — and voila! 3 perfect euploid embryos were created! We proceeded with FET with confidence — normal genetics and miscarriage treatments. We hit every milestone and celebrated graduation to the OBGYN! Nothing prepared any of us for what was to come. I remember the moment our front desk shared they had received a call from this patient that she needed to meet urgently. She had suffered a miscarriage at 17 weeks. The cause? Most likely a Parvovirus B19 infection that had circulated in her son’s preschool class. This is rock bottom 🥺 The patient underwent a D&E procedure and we met that same week — they needed to plan next steps. After a full recovery and brief testing update, we replicated the same protocol. We achieved implantation, and excellent BhCG rise, and graduation. Except this time, each milestone was paired with anxiety and fear — how could we possibly predict or prevent another fluke event? It was not until this Wednesday evening, when this little princess made her beautiful debut, that everyone FINALLY celebrated with relief this little miracle 🫶🏻

🩷🩷Happy Graduation to this very very special Twin Girl Mom in the making 🩷🩷 !!!!
Our journey started in 2019 — not quite ready to build her family, and with a prosperous career, she froze eggs to preserve future fertility and open up options for the future. Fast forward a few years and this patient returned — while she had not yet met “the one,” she now felt ready to pursue motherhood 🫶🏻 This decision can be so challenging and requires strength and bravery — but she knew she was ready. 💪🏼 Prior to using her frozen eggs, we decided to generate fresh embryos given her age and ovarian reserve — somehow better than when we had undertook egg freezing! Thus, we could save her prior eggs for possible future partner but also pursue her dream of becoming a mother 🫶🏻 Our first transfer attempt unfortunately resulted in “pregnancy of unknown location” and ended in ruptured ectopic pregnancy requiring emergency surgery. This traumatic event set us back physically and mentally. At one point, she inquired about gestational surrogacy as she could not fathom facing such disappointment again 🥺 After a much needed reset, and focus on both physical and mental health, she bravely set forward on her journey again. We changed our regimen to fit her needs while being medically sound. To our incredible joy, our first beta level returned absolutely fantastic — already a better start than last transfer. We watched the hormone rise rapidly 🫣 We did a very early ultrasound (due to h/o ectopic!) and were absolutely shocked to see that our single embryo had split — she was having twins!!!! Fast forward five weeks to today — now at 10weeks of pregnancy, with both baby girls dancing on screen today (💃 💃 !!) we’re all finally breathing a little easier knowing we’ve met this important milestone of graduation….finally, SIX years in the making 🤯 So so so incredibly proud and happy for this brave patient who took her fertility and journey to motherhood into her own hands, and has already fought like none other to bring these beautiful little miracles into being 🩷🩷
