POV: new patient walked in with nails he hadn’t been able to cut in a while - thick, overgrown, and hard to manage on his own. Debridement first, then CN-U to start treating the fungal infection at the nail bed. This is exactly why you don’t wait it out. 🦶

POV: debriding eczematous skin and thickened nails, then treating plantar warts with salicylic acid - routine maintenance that matters even more on a foot with a prior transmetatarsal amputation, where every callus and lesion gets watched closely. Skin care is foot care. 🦶

This wound won’t close under constant pressure, so we remove the pressure. A metatarsal osteotomy repositions the bone driving the plantar wound, and an Achilles tendon lengthening corrects the underlying tightness forcing that pressure forward. Wound offloading, done at the source!
