A normal A1C doesn't always mean everything is normal.

A1C is a valuable tool for assessing long-term blood sugar, but it's only one piece of the metabolic health puzzle. Factors such as red blood cell lifespan, iron status, inflammation, and certain medical conditions can influence A1C, meaning it may not always reflect what's happening beneath the surface.

If you're experiencing persistent fatigue, difficulty managing your weight, brain fog, or other signs of metabolic dysfunction despite a normal A1C, it's worth having a conversation with your healthcare provider about whether additional evaluation is appropriate.

Depending on your individual health history, that may include looking at markers such as fasting glucose, fasting insulin, triglycerides, HDL cholesterol, post-meal glucose, or other assessments of insulin resistance.

A1C is an important marker but it's only part of the picture.

For many people, A1C is the first number they hear when talking about diabetes or blood sugar, and while it's a valuable tool, it doesn't tell us everything about metabolic health.

A1C estimates your average blood sugar over the previous 2–3 months, but it doesn't directly measure insulin resistance, which is the metabolic dysfunction that often develops years before type 2 diabetes is diagnosed.

That's why two people can have the same A1C and very different metabolic health.

Looking at additional information—such as fasting glucose, fasting insulin, triglycerides, HDL cholesterol, and how your body responds to meals—can provide a more complete picture. Your healthcare provider can help determine which tests are appropriate based on your individual health history.
