Does the first provider matter?

Healthcare spending for spine pain exceeds $134 billion each year in the United States.

A 2024 systematic review analyzed 44 studies comparing chiropractic and medical management for adults with spine-related musculoskeletal pain.

One of the most interesting findings wasn't simply that healthcare costs differed.

It was that downstream healthcare utilization differed.

Patients who initially consulted chiropractors were consistently less likely to receive:

• Opioid prescriptions
• Advanced imaging
• Specialist referrals
• Emergency department visits
• Hospitalizations
• Surgery

This doesn't suggest that these interventions are never appropriate.

Instead, it highlights an important concept in value-based healthcare: The first clinical decision may influence the entire trajectory of care.

Choosing guideline-concordant, conservative management early may help reduce unnecessary escalation while ensuring patients receive the right care at the right time.

How patients enter the healthcare system may matter more than we think.
