A hospice plan of care isn’t created once and forgotten.

The interdisciplinary team meets regularly to discuss:

• Pain and symptom changes
• Medication effectiveness
• Emotional and spiritual needs
• Personal goals and care preferences
• Equipment or supply needs
• Caregiver concerns and support
• Changes in the patient’s condition

This ongoing collaboration keeps care personalized, responsive, and aligned with what matters most to each patient and family.

Nurses, physicians, CNAs, social workers, spiritual counselors, volunteers and bereavement professionals work together to support each patient and family physically, emotionally and spiritually.

🌟 Our August My Why story comes from Kristy Naylor, Director of Clinical Services at our Kalispell agency.

Her hospice journey began while working at an assisted living facility on Maui, where partnering with hospice nurses reminded her why she became a nurse in the first place. What she thought would lead her to a master’s degree instead led her to her calling.

Today, she carries the lessons and stories of hundreds of patients and families with her—along with a nursing legacy that began with her great-grandmother in Eastern Montana more than a century ago.

Thank you, Kristy, for leading with compassion, supporting your team, and helping our patients and families feel cared for and never alone.
