Seven consecutive ‘A’ grades demonstrates MHMC’s dedication to patients.
Montclair Hospital Medical Center (MHMC), a member of Prime Healthcare, was awarded an “A” Hospital Safety Grade for the 7th consecutive grading cycle by The Leapfrog Group, a nationally-recognized independent nonprofit focused on patient safety. Only 15% of eligible hospitals in Spring 2026 have achieved an “A” grade for at least five consecutive grading cycles.

“At Montclair Hospital Medical Center, patient safety is our highest priority and drives every decision we make,” said Gail Aviado, CEO of Montclair Hospital Medical Center. “We are proud to have earned our fourth consecutive ‘A’ grade from The Leapfrog Group, which demonstrates the dedication of our entire team to providing a safe, healing environment for our patients.”

The Leapfrog Group assigns an “A”, “B”, “C”, “D” or “F” grade to all general hospitals across the country using evidence-based measures of errors, accidents, injuries and infections as well as the systems hospitals have in place to prevent them.

“Montclair Hospital Medical Center deserves recognition for its unwavering focus on protecting patients and delivering safe care, earning an ‘A’ Hospital Safety Grade for seven consecutive cycles,” said Leah Binder, president and CEO of The Leapfrog Group. “Sustaining this level of excellence over time shows a true, organization-wide commitment to making patient safety a top priority every single day.”

The Leapfrog Hospital Safety Grade evaluates more than 30 performance indicators, including preventable errors, accidents, injuries, and infections, to assign a single letter grade to hospitals nationwide. Montclair Hospital’s continued “A” grade reflects its ongoing efforts to uphold rigorous safety standards and promote exceptional patient outcomes.

In addition to this latest recognition, Montclair Hospital Medical Center recently earned the Gold Plus Get With The Guidelines™ award from the American Heart Association for Heart Failure care and earned an “A” grade for Equity and Inclusivity from the Lown Institute. Indeed, Montclair Hospital Medical Center has consistently earned accolades for clinical excellence, including being a five-time recipient of the Healthgrades Patient Safety Excellence Award and has been named as one of the Top 100 Hospitals in the Nation by IBM Watson Health as published in Modern Healthcare.

The Leapfrog Hospital Safety Grade is peer-reviewed, transparent, and updated twice annually. It remains the only hospital rating system in the United States focused exclusively on patient safety.
