This week, we had both employees and residents test positive for COVID-19. As a result of contact tracing, should your loved one staying with us require a change of room or specific direct intervention, we will contact the responsible party directly. No news is good news. We continue to monitor daily and test multiple times per week..



Visitation rules:

Indoor visit

Visitor must be fully vaccinated; OR Visitor must produce a negative COVID test. If the negative test is a PCR test, it must have been taken with 48 hours of the visit. If the negative test is a rapid antigen test, it must have been taken within 24 hours of the visit.
Visitors must wear a mask at all times while inside the building
Brief contact is permitted only by visitors that are fully vaccinated
Visitors are limited to the visitation room or the residents room and the most direct path to/from that room.

Outdoor visit

Unvaccinated visitors must visit outdoors and maintain a six foot distance.
Unvaccinated visitors must wear a mask during visitation.
No contact permitted with resident.

Visitors going into a Yellow or Red Zone are required to wear full PPE, including an N95, gown and gloves.

Developing an individual, written care plan indicating the care to be given.
Supervise direct care of residents and nursing personnel on his/her shift which includes CNA staff.

Administer direct care as necessary. Ensure that nursing care is provided according to established standards and policies.

Ensure that all medications and treatments are performed and documented properly.
Administer medications by means of oral, external, injections, and intravenous methods and follow established policy & procedures for medication administration.
Administer treatments, including but not limited to; dressings, catheterization, suctioning, oxygen, skin assessments, etc.
Qualifications:

Must have an active, unencumbered licensed in the State of California as an RN or LVN. Must be able to pass a background check and drug screening.
Responsible for the overall nursing care on his/her shift. Provide necessary leadership by example to ensure that a strong sense of customer service and hospitality, as the overall goal, is maintained and executed by all staff at all times to both residents and co-workers alike.
Ability to read, speak and write the English language in order to understand instructions and communicate effectively with staff.

Excellent interpersonal and social skills in order to work effectively with residents, families, visitors, peers, supervisors, subordinates and other outside contacts.
Possess the ability to apply critical thinking to problem solve and function in a high volume, multi-tasking environment. Be a team player and work under pressure and handle conflict with ease.

The front desk receptionist is the first point of contact for residents, visitors and guests to Carmel Hills Care Center and therefore responsible for the first impression the community has of our facility. Every person entering the facility will be greeted by the receptionist.

QUALIFICATIONS

• Excellent customer relationship skills and positive personality.
• Excellent communication skills; able to speak, read and understand English.
• Minimum of 2 years experience in customer service.
• Proficient in Microsoft Office and able to learn other computing software as needed. Must also be familiar with other office equipment: copier, fax, and telephone.
• Must be able to lift a minimum of 20 pounds, climb small ladders, use step stools, reach for objects, stand and walk for periods of time greater than 20 minutes.

JOB DESCRIPTION

• Answer telephones on the first ring in a pleasant tone identifying both the facility and themselves while offering a greeting.
• Under the direction of the Business Office Manager, assist with accounts payable/receivable, payroll, filing and other duties.
• Assist other administrative staff with overflow work, including word processing, data entry and other internet tasks.
• Handles special administrative/department projects.
• Maintain the front desk in a neat, clean and orderly manner. Floors around the reception area and in the lobby will be swept as needed to eliminate debris that accumulates throughout the day.
• Maintain the front lobby area furniture, literature and trash that may accumulate.
• Manages the transportation schedule by verifying where appointments are located, if the van is available and ensuring there is sufficient time between appointments. Confirms appointments and availability with physician office and charge nurses or other staff as appropriate.

Plan, organize, develop, and direct Occupational Therapy Services in accordance with current applicable federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure that the highest degree of quality resident care can be maintained at all times.

QUALIFICATIONS

• Must possess, as a minimum, a Bachelor's Degree in Occupational Therapy from an accredited college or university.
• Must possess a current license to practice as an Occupational Therapist in California.
• Minimum two years experience in a therapist capacity in a hospital, skilled nursing care facility, or other related medical facility.
• Must have training in rehabilitative and restorative therapy practices.
• Must be able to lift a minimum of 20 pounds, reach for objects and stand and walk for periods of time greater than 20 minutes.

• Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly reviews.
• Brief resident of procedures involved in occupational therapy and prepare necessary equipment.
• Assist in transporting resident to and from occupational therapy room/area.
• Demonstrate to residents and staff personnel, as necessary, the procedures involved in the treatment process.
