Baby Boomers Recognition Day honors those born between 1946 and 1964. This generation has witnessed and influenced remarkable changes in culture, technology, work, family life, and communities.

As many Baby Boomers enter a new chapter, some are caring for aging parents while also considering their own health, independence, and plans for the future.

What is CAP/DA? For many families, it may offer a way to help a loved one remain safely at home. 🏡

The Community Alternatives Program for Disabled Adults (CAP/DA) is an NC Medicaid program that may help eligible adults age 18 and older receive approved services and coordinated support at home instead of moving into a nursing facility.

At PHC Home Health, our CAP/DA Case Management team works closely with participants, families, and providers to coordinate services and provide individualized, ongoing support throughout the process.

We help make the process easier to understand so families can focus on what matters most.
