Finding the perfect treatment for perimenopause and menopause is a lot like finding the perfect pair of jeans.

The first pair you try on isn’t always the best. Sometimes the second or even the third doesn’t quite work either. But you keep trying until you find the style, fit, and size that works for you.

And just because a particular style of jeans is trending doesn’t mean it’s going to be the most flattering—or the most comfortable—for your body.

Menopause care is much the same.

There is a huge push right now for bioidentical hormone therapy, and I’m a huge fan of this when it’s appropriate. It is often my first line treatment. But it’s important to remember that it isn’t the only approach, and it isn’t the right choice for every woman.

There are different hormone therapy options, different ways to deliver them, and nonhormonal treatments that can also help with certain symptoms. The goal isn’t to follow whatever menopause treatment happens to be trending. The goal is to find the treatment that fits you—your symptoms, your health history, your goals, and your preferences.

In other words, menopause care shouldn’t be one-size-fits-all.

You deserve a treatment plan that fits you—not one that simply happens to be fashionable right now.

Just kidding… kind of.

Behind every doctor trying to help women with hormones, metabolism, energy, sleep, weight, libido, perimenopause, menopause, and long term health is also a human being running on coffee, chaos, and a very strong desire to help people feel better.

Natural” does not automatically mean safer, better, or more effective.

Clean eating is great. Supplements can be helpful. Lifestyle matters. But real medical conditions like hormone imbalances, insulin resistance, thyroid issues, menopause symptoms, metabolic changes, and chronic health concerns sometimes need more than the “natural route.”

At MALI, we are not here to scare you away from medicine or sell you on trends.

We believe in personalized care that looks at what actually works for your body, whether that includes lifestyle, labs, prescriptions, supplements, hormones, or all of the above.

Because your health deserves more than “just do it naturally.”

Natural is not always better.

And no, you cannot always clean eat your way out of real medical conditions.

Nutrition matters. Movement matters. Sleep, stress, sunlight, and lifestyle all matter. But so do labs, hormones, medications when appropriate, prevention, metabolism, genetics, risk factors, and actually understanding what your body needs.

Sometimes the “natural route” is supportive. Sometimes it is not enough. And sometimes avoiding medical care for too long makes things worse.

Can wild yam cream turn into estrogen in your body?

Short answer: No.

Wild yam contains a plant compound called diosgenin. You may have heard that diosgenin is used in laboratories to make steroid hormones such as progesterone and estrogen.

But here’s the important distinction: your body does not have the enzymes needed to convert diosgenin into estrogen or progesterone.

So applying wild yam cream to your skin does not mean your body is making estrogen from it.

That’s different from a hormone cream that actually contains estrogen or progesterone.

When it comes to hormone health, it’s worth looking beyond marketing claims and asking: What is actually in the product, and what does the evidence show?

I may specialize in helping midlife women, but I definitely don’t discriminate—I love treating the men, too. 💪

One of my favorite longevity exercises? The farmer’s carry.

Why? Because it trains multiple systems that naturally decline with age: grip strength, balance, core stability, posture, and overall functional strength.

This isn’t just about building muscle—it’s about preventing frailty, reducing the risk of falls, and staying independent for decades to come.

My goal isn’t to help people simply live longer. It’s to help them live better. I want my patients playing with their grandkids in their 80s, traveling the world in their 90s, and enjoying the life they’ve worked so hard to build.

That’s what longevity is all about. And sometimes, it starts with something as simple as picking up heavy things and walking. 🚶‍♂️🏋️‍♀️

Influencers on social media: “Here’s the one thing that fixed my hormones.”

Doctors on social media: “Actually, it depends on your symptoms, labs, history, medications, risk factors, lifestyle, metabolism, sleep, stress, and what your body actually needs.”

Not as catchy, but definitely more helpful.

GLP-1s are not magic. They don’t build healthy habits for you. They don’t prepare your meals, make you strength train, improve your sleep, manage your stress, optimize your hormones, or undo decades of metabolic adaptation. They are a tool. For many people, they’re the tool that finally allows all the other pieces of the puzzle to work.

Can we also stop reducing obesity to the tired mantra of “calories in, calories out”?

Yes, energy balance matters. But pretending that’s the entire story ignores everything we’ve learned about physiology. Hunger hormones. Satiety signaling. Insulin resistance. Leptin resistance. Genetics. Stress. Sleep deprivation. Menopause. Testosterone. Cortisol. Thyroid function. Gut hormones. Medications. Inflammation. The brain’s reward pathways. These aren’t excuses—they’re biology.

Weight regulation is one of the most complex systems in the human body. Reducing it to “just eat less and move more” is like telling someone with depression to “just cheer up.” It’s an oversimplification that ignores the science.

This has never been about willpower.
That isn’t a character flaw. It’s biology.

When GLP-1 medications are used appropriately—as one part of a comprehensive plan that includes nutrition, exercise, muscle preservation, sleep, stress management, hormone optimization when indicated, and sustainable lifestyle changes—they can be absolutely life-changing. Not because they’re “easy,” but because they help correct some of the biological signals that have been working against people for years.

That’s not cheating.
Stop shaming people for using the tools available to them.
