At SouthernCare Hospice, we believe we’re Better Together.🩵

One of the ways we strengthen our partnerships is through Better Together meetings working alongside facility teams to identify needs, support staff, and make sure patients and families receive the best care possible.

Hospice can help fill the gaps in care by providing an extra layer of support through skilled nursing, symptom and pain management, hospice aides, social work, chaplain services, medications and equipment related to the hospice diagnosis, and 24/7 on-call support.

Our goal isn’t to replace the incredible care facilities already provide it’s to come alongside their team, lighten the load, and provide additional support when patients need it most.

Because when we work together, our patients and families receive better care. 🤝🩵
