*EDIT: Due to the system outage, we are unable to access patient information to be able to notify patients until the system is accessible again. We are not sure when that will be at this time. As soon as the system is back up and that information is accessible, we will reach out to patients regarding the situation to the best of our ability. In the meantime, please leave a detailed voicemail if you need to reach our office, keeping in mind that we are limited in how we are able to help until the system is operational again. We sincerely apologize for the inconvenience and understand that the timing of this nationwide system outage is incredibly frustrating for our patients. We appreciate your patience and understanding.

We hope to re-open for one last day tomorrow, 4/30/26, but will post an update if system concerns continue.

We are sad to announce that Millennial Medical Clinic will be closing its doors on 4/30/26.

We hope the slides included here will address many of your questions, and we will continue to update the Closing FAQs page on our website as new recurring questions become clear.

Please be patient with office staff during this transition period, as we expect unusually high call volume and appointment demand following this announcement and have limited personnel available to respond. Calls will be taken in the order in which they are received, and appointments will be availability-based and extremely limited. We sincerely apologize for this inconvenience and appreciate your patience and understanding.

We are so grateful to every patient and staff member who made this clinic what it has been for the past nearly 8 years. It has been an honor to provide holistic primary care in this special community.

Patients who wish to continue receiving holistic primary care from Madison Delk, PA-C after our office closes can contact Covenant Primary Care Murfreesboro at (615)-239-2062.

Appointments currently scheduled with Madison Delk after 4/30/26 have been automatically transferred to Covenant Primary Care to ensure they are held for the patients who had reserved those times with her during our time in business. If you do not wish to continue care at this location, please notify our office or cancel in your Healow app.

Thank you to each one of you for visiting our office, recommending our providers, bringing homemade treats, loving our kids, and supporting our cause.

In such a disconnected world, our staff and patients alike felt like family, and we do not take that for granted.

We are heartbroken with you that this chapter is coming to an end, and we remain hopeful that our paths will cross again in the next one.

Patients and staff members alike have been known to call her superwoman, and it's no secret why. She shares her deep knowledge and warm spirit with every patient that comes through our doors. From serving the community in clinic during the day to being the most amazing mom as soon as she gets home, Madison is truly an inspiration to all of us, and one we are happy to celebrate today.
