Fragmented care has a cost that rarely gets named directly: it doesn’t just miss diagnoses, it misses the person entirely.

Progress isn't always linear, and sometimes the body tells a different story than the symptoms do.

I've seen this pattern more than once. Someone's labs clear up, their physiology genuinely improves, and yet their symptoms don't follow. That gap usually isn't physical. It's something the DTT™ framework has taught me to look for underneath the surface: whether someone actually believes they're allowed to get better.

Self-worth shapes healing in ways that rarely get named in a typical appointment. It shows up in who follows through on the next step and who quietly stalls, even when the resources are there.

Day 5. Time doctors are "allowed to spend" with patients and for their wholesome care.

trial showed us exactly how much time her "providers" were spending with her, on her care, with her family, etc. and, rightfully, the providers are criticized for it.

but here's the truth - They're all following "standard of care" as set by APA guidelines, their health care admins, and their insurance companies. when diseases are seen as "chemical imbalance", then all you need to do is prescribe. Why would anyone pay attention to nutrition, stress, toxin load, support system?

I faced immense pressure to see more and more patients as time went by. To become more "efficient", to not keep patients admitted too long because hospitals make more money with admissions than with long stay for a patient, etc.

All this influenced how I run my practice. Since 2016, my intake appointments are 90 min - 60 min face to face with patients, then 30nmin for me to think, process and formulate my treatment plan for them. I routinely ask to speak with family members and invite them in sessions. follow up appointments are 25 or 50 min long.

As doctors, we need time to get to know who you are. that is if the goal is to heal the person and not just manage the symptoms.

Day 4 - Continuing with my series - "Real Conversations I have had as a psychiatrist, mom and woman."

In this video I shared an incident when I was reprimanded for not giving an Alcohol dependent patient op**tes for his pain. I was told I was attracting negative reviews and the hospital will suffer for it. But this was the same medical director who told me that if ever his loved one was sick, he trusted only me to care for them.

I had conviction in my analysis, thinking, and clinical judgment. I could withstand this pressure (for a few years). And then I quit to start a practice in alignment with holistic healing - Trinergy Health was born in 2016.

So when people question the care was given, I have to repeat that she received "standard care." As dictated by hospital systems, insurance companies, and Big pharma.

is making us question psychiatry and the medical industrial complex in general.

bottom line, health is created with our daily choices. No outside entity can give us our health.

Day 3 - Op**te edition.

We know why "pain" became a 5th vital sign. Icymi, Op**te manufacturers "educated" doctors and health care systems as to how "important" it was to manage pain and to evaluate pain for every patient. Doctors and systems were "educated" to prescribe Op**tes liberally and ignore the real risks of addiction. I saw the op**te epidemic unfold and I did not participate in it. But the pressure to conform was relentless.

For this op**te epidemic, I hold physicians and health systems just as responsible. and insurance companies. The pressure to prescribe also came from patients and the scourge of "online ratings". Truth is there is no pill that can take away our suffering.

We see a similar process with psych drugs. These drugs are toxic and doctors should pay attention to how these drugs are affecting their patients and become conservative with prescribing. Lifestyle medicine should become the bedrock of any intervention.

Hopefully, trial will awaken people to the horrors of psych drugs.

I have not evaluated Lindsay Clancy and I have not seen her records. This is my reading of what’s publicly available.

I don’t think this was postpartum psychosis, which puts me at odds with her defense.

Psychosis that arises on its own has a shape. Gradual decline, and everyone around the person sees it coming. Lindsay presented as articulate and rational to every provider she saw, while telling them the medications were causing thoughts of harming her children.

That disconnect is the whole case. Thirteen drugs in four months, and almost no workup underneath any of it.

Truth is stranger than fiction.

Day 1: I shared about my first day of psychiatric residency here in US (Please see that video first).

Day, 2: I am sharing about my first day of psychiatric restency at NIMHANS in India.

I was trained in both residencies with the same textbooks (and ICD 10 in India), yet the interpretation of these textbooks was vastly different. In India, I was taught that psychiatric diagnosis was a diagnosis of exclusion. In other words, we diagnosed a person with a psychiatric illness only after medical and neurologic illnesses, where ruled out as root causes.

I left India in 2004 and in 2005, I started my second psychiatric residence here in US. Right away, I was stumped with the complete divorce of mind and body within the practice of psychiatry in US.

Why and how did this happen? I think it is related to reimbursement structure with insurance companies dictating what is standard of care and what is reimbursed, which results in a cookie cutter approach to treat a diagnosis. The person gets completely ignored.

Add to this, big pharma propaganda, pushing the unproven narrative of chemical imbalance theories and treatment with psychiatric drugs.

The deep systemic problems in psychiatry are laid bare by trial.

I repeat: the type of care Lindsay received is standard of care.
