Some news I'm really proud of! Our oculoplastics program at Manhattan Eye, Ear & Throat Hospital is expanding thanks to a $1 million gift that has enabled us to open Northwell's first standalone oculoplastics practice. Using a lash serum? Check the label.

Many popular lash serums contain prostaglandin analogs—ingredients originally developed to lower eye pressure in glaucoma patients and later found to stimulate lash growth.

The tradeoff? With prolonged use, some patients develop changes around the eyes, including:

• Hollowing
• Dark circles

Fat grafting is one of my favorite procedures. I use it often, and in the right patient, it can produce beautiful, natural, long-lasting results.

But I’ve also cared for patients after fat grafting performed elsewhere, and that’s why I believe every patient deserves to understand both the benefits and the risks before making a decision.

If you’re deciding between under-eye filler and fat transfer, filler can sometimes be a reasonable place to start because it’s temporary. If the result isn’t quite right, hyaluronic acid filler can often be dissolved. Fat transfer is different. Once the fat has healed, there isn’t a simple “undo” button.

I’ve also treated patients years after fat grafting who developed fat nodules or granulomas—firm areas that can occur when transferred fat doesn’t heal as intended. They’re treatable, but they’re certainly not part of anyone’s plan.

The complication I worry is discussed far too little is vision loss. Although extremely rare, fat can enter a blood vessel and travel to the arteries supplying the eye, resulting in permanent blindness. It’s an uncommon event, but when it happens, the consequences can be devastating.

This isn’t meant to scare you away from fat grafting. It’s meant to help you make an informed decision with a surgeon who performs a thorough evaluation, understands the anatomy intimately, and is comfortable discussing all of the risks—not just the benefits.

The best procedure isn’t the one that’s trending. It’s the one that’s right for you.

Ptosis repair is one of my favorite procedures because the transformation goes beyond aesthetics and helps restore vision and confidence for my patients.

What makes my approach unique is that before we ever step into the OR, I can simulate results in the office. Using a specialized drop, I can temporarily lift the upper eyelid so you can see exactly what surgery can achieve. It's part of the custom algorithm I've developed to remove the uncertainty from eyelid surgery and help me understand how you may respond to surgery.

Fat grafting is having a moment—and for good reason.

I use fat grafting to restore volume in the midface, soften the tear trough and under-eye hollow, and even help reconstruct contour defects, such as the dents or divots that can occur after craniotomy or other craniofacial surgery.

What makes fat unique? It’s more than just a volumizer. Fat is living tissue that contains adipose-derived stromal cells, along with growth factors and other biologically active cells that may support tissue quality, healing, and skin health. While ongoing research continues to define these regenerative effects, fat grafting offers benefits beyond volume replacement that synthetic fillers cannot replicate.

I use fat grafting both as a standalone procedure and in combination with eyelid and facial surgery to restore natural contours and optimize long-term results. It’s one of the most versatile tools in facial rejuvenation—and one of my favorite procedures to perform.
