Pain management options for in-office GYN procedures—ranked by effectiveness
Sedation / anesthesia (IV or OR-level) — most effective; but appropriate for select patients
Paracervical (cervical) block — strongest in-office option for procedural pain
Topical anesthetics (lidocaine gel/spray/cream) — modest benefit, low risk
Procedure-specific adjuncts (e.g., misoprostol, vaginoscopy) — variable, situational benefit
NSAIDs (ibuprofen, naproxen) — helps post-procedure cramping not so much for intra-procedure pain
Anxiolytics — reduces anxiety, not pain directly
Supportive measures (education, trauma-informed care)
— improves experience and pain perception
Sometimes a combination of these options can improve effectiveness. My personal favorite is topical cream and anxiolytics with NSAIDs after the procedure.
