Your body didn't develop chronic pain because something went wrong.
It developed chronic pain because something went right.

When anxiety became your baseline, your fascia literally curled around your heart, chest collapsed inward, shoulders rounding forward, to protect you from a world that felt perpetually threatening.

That's the Shield pattern, and even though it might have developed into poor posture over time, it's actually intelligent armor.

When financial stress, unstable housing, or unpredictable relationships threatened your sense of security, your legs stopped trusting the ground beneath you. Misaligned, compensating, never quite able to receive support. That's the Foundation pattern, your physical instability mirroring the instability in your life.

These patterns formed to keep you functioning during impossible circumstances. They worked. And now they're the source of your pain.

Last weekend I found myself flat on the grass in Pennsylvania, face-to-the-sky, sun doing its thing, and this little guy curled right into me like I'd always been his.

He's not my dog. But somewhere along the way, we found each other. And every time we're together, something in me settles. He softens me. He reminds me what it feels like to just be, no performance, no agenda, no armor.

There's something that happens when you let an animal love you. It feels so real. The warmth of a body that isn't keeping track of anything aside from being right here and now. The way a dog looks at you like you are, simply, enough.

He lets me work on his little body, massage, realign, just listening with my hands. And I wonder sometimes if he knows something I'm still learning, which is that belonging isn't something you earn. It just is.

Being outside, on the ground, in relationship with a creature who lives entirely in his body is not separate from the work I do. That is the work. We are nature. All of us. And when we remember that, something settles within us and makes us feel like home.

So many people live in this state every day without realizing their body is stuck in survival mode.

But, your body isn’t failing you. It’s protecting you.
Chronic stress changes the body on a biological level. Increasing cortisol, dehydrated fascia, amplifying pain signals, disrupting sleep, and keeping the nervous system in a constant state of vigilance.

Over time, temporary protection can become a long-term Pain Pattern.

In sixteenth century England, women who spoke in ways their communities found inconvenient were fitted with an iron cage that pressed a plate down against the tongue.

The device fell out of legal use centuries ago, and yet I find I cannot stop thinking about it, because thirty years of working on the necks of women has taught me that the bridle never really left. It only moved inward, and what was once iron has become muscle.

This week on Substack, I am writing about what the female throat has been holding, what the anthropologists and the developmental psychologists were finding in the nineteen-seventies that the orthopedic surgeons still rarely read, and why women carry significantly more neck pain than men in every region of the world that has been measured.

Over twenty years of practice, I've noticed that most people arrive with one of seven distinct pain patterns.

Not random or mysterious pain but patterned pain shaped by how your nervous system has learned to hold you together against the accumulated pressures of a lifetime.

To make this more accessible, I created a language for these patterns, rooting each one in the first seven sessions of the Structural Integration series developed by Dr. Ida P. Rolf. Today, I'm sharing all seven for the first time in a slideshow I made to help you see yourself in them.

As you move through the slides, notice what resonates. Most of us carry one pattern as predominant, though you may find yourself in more than one. That's not unusual. We are complex beings, and our bodies reflect that complexity.

Here's what I've come to understand: nothing arises out of the blue. As we age, our bodies begin to show us the accumulation of years of unconscious holding. Habits, injuries, surgeries, tension, emotion, trauma, beliefs. The nervous system responds to all of it, molding fascia into shapes that keep us functioning until, gradually, they don't.

Structural Integration is one of the most effective approaches I know for releasing these patterns through precise, attentive, hands-on work. If you can make it to my office in New York City, I'll go further still helping you identify what emotions may be moving alongside your physical patterns, so you can experience a full body-mind-heart connection.

We are complex beings. The body is our primary mode of transportation. The mind, our compass. The heart, our connection to love. And the spirit, an energetic lifeline to everything mysterious and alive.

This is what recovery looks like.

I had a bunionectomy. And this is the slow, unglamorous, necessary work of getting my foot back.

Video 1. A resistance band under the ball of my foot. What you're seeing, that shaking, is my fascia reminding me where I still hold stiffness. There's still inflammation. Still some pain at the big toe joint. But I'm moving through it. Patiently. And speaking to my foot with kindness. Yes, really. The body is always listening.

Video 2. Now I'm asking each toe to move independently, something that used to happen automatically. That shaking is my nervous system re-learning. Full extension of the big toe joint triggers the Windlass Mechanism, the moment the foot transforms from a flexible shock absorber into a rigid lever for pushing off. My push-off is not at its best right now. So I go back to basics.

Video 3. Here I'm pressing into a balance pod to reawaken the dome of my foot. Think of a camera tripod, three points of contact creating a perfectly stable base. Your foot works the same way: the ball of the big toe, the ball of the pinky toe, and the heel. When weight distributes evenly across all three, you have stability. And with over 8,000 nerve endings in the soles of your feet, this isn't just mechanics. It's your body's conversation with the ground. I'm learning to listen again.

Video 4. A resistance band around both big toes, gently pulling them apart. This is both a passive stretch and an active strengthener, counteracting the inward pull that creates bunions, reactivating the deep intrinsic muscles of the foot, and restoring the natural splay of the toes that years of narrow footwear trains right out of us.

Simple tool and a great deal happening underneath.
This is what I do. I work with the body, not against it. One steady step at a time.

Your stress response was designed to save your life. But it was never meant to stay ON all the time.

When stress becomes chronic, the body shifts into long-term survival physiology. Your muscles stay tense, inflammation increases, sleep becomes disrupted, digestion changes, pain sensitivity rises and fascia begins holding protective tension patterns.
Over time, the body adapts to unresolved stress by creating patterns of protection. You may notice your shoulders lifting, jaw clenching, breath shortening or posture collapsing inward.
And eventually those patterns start to feel “normal.”
This is one of the reasons chronic stress so often becomes chronic pain.
In this week’s newsletter, we explore the neuroscience behind stress, fascia, pain, and nervous system dysregulation and why true healing often requires more than stretching, strengthening, or simply “pushing through”.
When you understand what your body is trying to protect you from, you can begin working with your nervous system instead of against it.
