Here at CCHP, we can get you connected to the resources you need through the Social Care Network (SCN).

In order to receive these services, you must:
Have active Medicaid
Live in Manhattan, Brooklyn, or Queens
Connect with one of our patient navigators to do a screening to see if you qualify for services

At CCHP, we provide testing, treatment, and medical case management for those infected or at risk, as well as preventive vaccines for Hepatitis A and B, and the cure for Hepatitis C.

This week, we had a great visit from Reach Out and Read of Greater NY and the Carmel Hill Fund. 📖

With Reach Out and Read, we’re able to pair children that come in for a pediatric well-visit with a free book at their reading level. ROR helps to develop reading skills and bring families together. 📚

With the support of Carmel Hill Fund, whose mission is investing in the future of NYC youth, CCHP has helped 187 families grow their home libraries in the last year, and since joining Reach Out and Read of Greater New York in 2014, we’ve given out 2,467 books for children aged 6 months to 5 years old! 🎒

When children learn to read, their own story begins.

Thank you to Reach Out and Read of Greater NY and the Carmel Hill Fund for helping us to best serve the families that call CCHP their home for a lifetime of care! 🫀

Drag queen and community advocate Brita Filter talked to us about the importance of sexual health at National HIV Testing Day last month!

is a performer, activist, and cast member of RuPaul’s Drag Race who has made her mark on NYC’s LGBTQ+ communities and advocacy spaces. 👑

We’re proud to stand alongside advocates like Brita and community orgs like Callen-Lorde to continue our decades-long tradition of comprehensive, compassionate, community-based care around HIV/AIDS and overall sexual health. 🤝

Thank you to Callen-Lorde Community Health , NYC Health , and Gilead Sciences for having us at the National HIV Testing Day event, to help bring HIV testing and sexual health awareness to the people!

Legionnaires’ disease has recently impacted Upper East Side neighborhoods. The disease can lead to serious complications or death, but can be treated if diagnosed early.

Legionnaires’ disease causes a flu-like illness that may be severe in older adults, those with respiratory conditions, and those with a compromised immune system.

The bacteria that cause the disease live in warm water and mostly spread through humid air and cooling towers.

Legionnaires’ is not contagious: you cannot catch the disease from another person, drinking water, cooking, or air conditioners.

Common symptoms:
- Fever
- Chills
- Muscle aches
- Cough
Less common symptoms:
- Headaches
- Fatigue
- Loss of appetite
- Confusion
- Diarrhea

If you or anyone you know experiences these symptoms, seek medical attention as soon as possible.
