French Creole individual to provide direct support for families at home to manage their chronic illness by assisting and guiding them with health care, advocacy, medications, doctors appointments and follow ups, housing, prescriptions, social security, medicaid and many other daily living issues they may face.

We provide services for children and adults with chronic health conditions to give them the best support necessary to manage their condition such as Asthma, Heart problems, Hiperlipidemia (Cholesterol), Autism Spectrum Disorder, Hypertension, HIV/Aids, Developmantal Desabilities, Chronic Hepatitis, Depresion, Dementia, Kidney disease, and many others. We work with individulas speaking several other languages other than English specially Spanish and French Creole. We have workers able to communicate with them in their language.
