🩺 Knee Dislocation: Part 1 of 3

When I talk to patients about a knee dislocation, one of the first things I explain is that not every “dislocated knee” is actually the same injury.

Sometimes it’s the kneecap that has moved out of position. A true knee joint dislocation, however, is much more serious and can involve several important structures at once.

That distinction matters because the treatment and the urgency can be very different.

In this first part, I’m breaking down what a knee dislocation actually is. In the next two reels, I’ll go deeper into the injuries it can cause and how I evaluate and treat them.

I’m Dr. Armin Tehrany, and for me, orthopedic care has always been more than just treating an injury. It’s about helping people get back to the life and activities they love.

I’m an orthopedic surgeon at Mount Sinai and see patients at my private practice on Madison Avenue in Manhattan. Within orthopedic surgery, I specialize in minimally invasive surgery of the shoulder and knee.

What matters most to me is taking the time to understand each patient, explain their options clearly, and find the right path forward together.

Harder cases aren't a liability. They're a responsibility.

Every surgeon has cases they could reasonably decline. Complex revisions. High-risk anatomy. Patients who've been told elsewhere that their situation is ""too complicated.""

I take most of them.

Not because I'm reckless — but because I think walking away from complexity, when you have the training and the tools to address it, is a failure of what this profession is supposed to be.

The patients who come to us after being turned away somewhere else aren't problems. They're people who needed a surgeon willing to think harder, plan more carefully, and commit fully.

That's what I signed up for.

The best surgeons I've trained all share one trait. And it has nothing to do with technical skill.

I've taught surgical technique to residents and fellows for years. And at a certain point, technical skill becomes a baseline — everyone who gets to that level can operate.

What separates the exceptional ones is something harder to teach: the ability to sit with uncertainty, ask the next question, and resist the urge to act before they fully understand.

The pressure in medicine — especially surgery — is to move fast, decide quickly, book the case. And sometimes that's right. But the surgeons I've watched become truly excellent are the ones who learned to slow down at the decision point, even when the system is pushing them forward.

That habit — intellectual honesty under pressure — is what I try to model. For my trainees, and for every patient who sits across from me.

The answer to "should I get a knee replacement?" should never be driven by what's fastest to schedule.

This is one of the most consequential decisions a patient will make. And yet it's often made in a 15-minute appointment, driven by imaging alone, with little discussion of alternatives.

It wasn't about the money. It was about what I couldn't give my patients inside it.

In a large hospital system, you see a lot of patients. You order a lot of tests. You wait for a lot of approvals. You refer to a lot of committees.

And somewhere in there, the patient — the actual human being in front of you — becomes a case number moving through a process.

I didn't become a surgeon for that.

I started Manhattan Orthopedic Care because I believed there was a better way to practice: fewer patients, more time, faster decisions, total transparency. A place where I could actually do what I trained to do — not what the system had time for.

It's a harder model to run. But every time a patient tells me they can't believe how fast we moved, or how clearly we explained their options, I know it was the right decision.

This is what medicine should feel like.

🩺 Patient Recovery Story

This patient’s journey highlights how recovery can sometimes involve more than one step—and patience along the way.

After first injuring her knee and undergoing surgery, she gradually worked through the recovery process. While arthritis created a few setbacks, consistent care and rehabilitation helped her regain strength and stability.
