Manhattan Pain Medicine, PLLC

Manhattan Pain Medicine, PLLC Pain Management and Physical Rehabilitation with concierge-level care. We also treat the complications of brain injury, stroke, and spinal cord injury.

Manhattan Pain Medicine offers complete and individualized care for those with pain or functional limitations. We treat challenging and painful conditions – such as headache, fibromyalgia, abdominal pain, neck or back pain, cancer pain, or amputation pain (phantom limb pain) – with careful consideration and innovation. The goal of the initial evaluation is to determine the precise cause(s) of pain

or functional impairment. Our treatments then aim to fix the source of the problem, promote healing, prevent recurrence, and restore quality of life. We offer a uniquely broad range of treatment options. These can include medications, physical or occupational therapy, injections, botulinum toxin (Botox®), changes to diet, alternative medicine, and more. For those who have found themselves on large doses of opioid medications, we specialize in treating pain while weaning opioids without discomfort, including the use of Suboxone® for those with difficulty coming off opioids. All your care will be handled by Dr. Jason Siefferman, a board-certified specialist in Physical Medicine & Rehabilitation and Interventional Pain Management, and Clinical Assistant Professor in the Department of Anesthesiology, Division of Pain Medicine at New York University. Dr. Siefferman completed his training at Rush Medical College in Chicago and Mt. Sinai School of Medicine in New York and scored in the top 1% nationally on his board certification exam. In addition to Manhattan Pain Medicine, he currently works in the VA Hospital system, treating veterans with a wide range of painful conditions. Manhattan Pain Medicine, PLLC was founded with the exact mission of providing cutting-edge care without the restrictions of commercial insurance. For this reason, we are happy to bill your insurance out-of-network, but we are not in-network for any insurance. We believe that the cost of medical care should be reasonable, and have set rates that welcome the uninsured. The cost of an office visit is based on the amount of time you spend with the doctor, and any additional procedural costs are discussed in advance. The result is that our patients know exactly what a visit will cost and they receive unparalleled care. Our patients also receive additional concierge-level services, such as direct access to your physician via cell phone, online access to your medical records, and home visits and videophone follow-ups.

What if the place that hurts isn't actually the problem?

As Kira Charles explains, the body works as an interconnected system. Pain often shows up in one area, but the underlying cause may be how other parts of the body are moving, compensating, or carrying extra effort.

Sometimes the solution isn't pushing harder—it's moving differently.

If an injury keeps coming back, it may not be because you're doing something wrong.

As Dr. Kane explains, many chronic injuries involve tendons and ligaments, which naturally have limited blood supply and can be slow to heal. The pain may improve, but if the tissue hasn't fully recovered, symptoms often return when activity increases.

The goal isn't just temporary relief—it's helping the tissue regain strength, stability, and function.

What if your "back pain" isn't actually coming from your back?

As Dr. Ahmed explains, the pelvic floor is closely connected to the hips, spine, core, and abdomen. When those muscles aren't functioning properly, pain can show up in places that seem completely unrelated.

If you've tried traditional treatments and your pain keeps coming back, it may be time to look beyond the lower back.

Pain isn't always as simple as finding one injury and fixing it.

As Dr. Aranguren explains, chronic pain often develops layers over time—from compensations in the body to changes in how the nervous system processes pain. That's why our approach starts with understanding the whole picture, not just the symptom.

Discovery. Treatment. Maintenance.

Because lasting relief starts with understanding what's really driving the pain.

When symptoms flare, it's natural for the brain to jump to the worst-case scenario.
But as Dr. Barbiere explains, there's a difference between catastrophizing and planning.
Instead of asking, "What if this gets worse?" try asking, "If it happens, what's my plan?" Small shifts like this can help reduce fear, lower stress on the nervous system, and make symptoms feel more manageable.

The goal of pain management isn't to find one stronger medication.

It's to build the right combination of treatments so you can use less of each while getting better results.

At Manhattan Pain Medicine, we use a multimodal approach—combining medications, procedures, movement, rehabilitation, and other therapies to target pain from multiple angles while prioritizing safety and function.

06/19/2026

Pain isn't always caused by inflammation alone.

Sometimes the real issue is tissue instability—when ligaments, tendons, and connective tissues aren't providing the support your joints need. This is especially common in patients with hypermobility, recurrent injuries, or conditions like hypermobile Ehlers-Danlos syndrome.

The key is understanding *why* the tissue is painful and what role it plays in stability, movement, and function.

Pain shooting from your lower back down your leg isn't always "just back pain."

Sciatica often happens when a nerve becomes irritated or compressed in the spine, commonly from a disc herniation. Understanding the source of the pain is the first step toward choosing the right treatment and getting lasting relief.

Pain is rarely just about the original injury.

At Manhattan Pain Medicine, we use a Discovery → Treatment → Maintenance approach to understand what's driving your symptoms, create a personalized treatment plan, and help you build lasting improvement. Because the goal isn't just pain relief—it's understanding the why behind the pain.
