This is one of the first questions men think about… and one of the last ones they feel comfortable asking.

S*x after prostate cancer can change, and for many men, that part of the conversation is either rushed or skipped entirely. So what’s left is uncertainty, frustration, and the assumption that it just won’t be the same again.

But s*xual function doesn’t simply disappear. It evolves, and in many cases, it can be supported and improved with the right approach.

Recovery takes time. Expectations need to shift. And most importantly, it requires actually talking about it.

A lot of women assume it’s just part of getting older, being busy, or something they’re expected to push through. So it never gets brought up, and never gets properly addressed.

But changes in desire or s*xual function are not random. They’re often connected to what’s happening hormonally, neurologically, and systemically in the body.

And when you actually look at it through that lens, it becomes clear this isn’t something to ignore… it’s something worth understanding.

Most men assume changes in libido or performance automatically point to testosterone.

But one of the most common patterns I see is chronic stress quietly driving the issue.

When cortisol stays elevated, your body shifts into a state that prioritizes survival over everything else. Over time, that shift can interfere with hormonal balance, energy, sleep, and overall responsiveness.

S*xual function is one of the first things to take a hit because it depends on your body being able to relax, respond, and stay regulated.

If things feel off, it’s not always about adding more or jumping straight to treatment. Sometimes it’s about looking at the bigger picture and addressing what’s keeping your system under constant pressure.

Lifestyle factors influence s***m production, motility, morphology, and DNA integrity long before fertility becomes a concern. That’s why changes in sleep, nutrition, stress, physical activity, heat exposure, and toxin load can meaningfully alter semen parameters over time.

From a clinical perspective, abnormal results are often a signal to look upstream, not just at reproduction but at overall health. When lifestyle inputs improve, s***m quality often follows.

If you’re thinking about fertility now or in the future, this is the window where intervention matters most.

There’s a lot of confusion and anxiety around prostate screening, so when I was asked to contribute to this Men’s Health article, I welcomed the opportunity to address some of the most common misconceptions.

Much of the concern comes from not understanding what the blood work measures, why it’s ordered, or how results should be interpreted. This test looks at a protein produced by the prostate to help assess risk, not to give a yes or no diagnosis. Context matters, including age, symptoms, family history, and how values change over time.

Screening today is far more individualized and evidence based than many men realize. When done thoughtfully, it helps identify who needs closer evaluation and who can be safely monitored.

If you’ve been putting off screening or aren’t sure what’s right for you, this is exactly the type of conversation we have in my practice. Education comes first, decisions come second, and your personal risk always guides the plan.

S*x drive doesn’t disappear randomly. It changes when the biology behind it changes 🧠⚡️

Libido is influenced by s*x hormones, blood flow, sleep, stress, medications, and overall metabolic health. Testosterone, estrogen, and progesterone all play a role, but how your body responds to those hormones matters just as much as the numbers on a lab report.

When desire shifts, it’s often your body asking for attention, not something to ignore or power through.

If changes in libido, arousal, or s*xual satisfaction are affecting your quality of life, that’s a medical conversation worth having.

At our practice, we take a comprehensive approach to s*xual health, looking beyond one hormone to understand the full picture and build a plan that actually fits your body.

Erections are one of the most sensitive vascular functions in the body. That’s why diet shows up here first.

In a large JAMA Network Open study, men with higher adherence to a Mediterranean-style diet had a significantly lower risk of erectile dysfunction, even after adjusting for age, activity, and smoking. The biology tracks: endothelial dysfunction, impaired nitric oxide signaling, insulin resistance, and inflammation all reduce pe**le blood flow long before they cause overt heart disease.

As a urologist, this matters because erectile dysfunction is often an early warning sign, not an isolated complaint. Pills can improve symptoms, but lifestyle and vascular health determine trajectory.

If erections have changed, it’s worth asking why. A proper evaluation should look beyond testosterone and prescriptions to cardiometabolic risk factors that can be modified early.

Aging influences hormones, but function tells the full story. In clinical practice, the focus is not age alone, it’s how well the body’s systems are working together over time.

Testosterone reflects the health of multiple systems, including:
• Sleep and circadian regulation
• Metabolic and insulin signaling
• Body composition and muscle mass
• Inflammatory load
• Cardiovascular and vascular health

When these systems are supported, men often maintain strength, energy, cognition, and s*xual function well into later decades. When they’re disrupted, symptoms appear long before testosterone falls outside a lab reference range.

If something feels off, don’t assume it’s just age. A thoughtful evaluation can clarify what’s normal, what’s modifiable, and what deserves treatment.
