What an honor to host Dr. Marco Bigoni, Dr. Marco Turati, and Dr. Marco Crippa from Milan, Italy, for a day and a half in the OR to see the Preservation First approach to ACL injuries up close. 🇮🇹

We had an incredible first day, and a first for me: we hit the full RBAR+ cycle! Across five ACL cases, I performed a repair with anchors, a repair with rings, a repair with BEAR Implant 🐻 Miach Ortho , a repair with augmentation, and a reconstruction. I’ve never had a day where I performed every variation of the approach.

That evening, we continued the conversation over a great steak dinner with Dr. Dan Green, Dr. Kyle Kunze, and my research fellow, Dr. Christian Arras.

The next day brought a hospital tour and time to explore the city, followed by cocktails on the rooftop of the New York Athletic Club with a perfect sunset view over Central Park. We wrapped up with a half day at the HSS North Jersey ambulatory surgery center, where I performed two more repairs and another repair with BEAR.

Across the eight ACL patients treated this week, 75% did not need a graft. And the word about Preservation First is reaching far beyond New York and New Jersey. Our patients came from England, Peru, Grenada, California, and across the New York and New Jersey area.

A memorable few days, great conversations, and one very full RBAR+ cycle.

Dr. Fealy and I may be teammates at Hospital for Special Surgery, but on game day, it’s Columbia vs. Princeton.

Thankfully, the Tigers came out on top at Baker Field this year.

Today, we’re back on the same team at HSS, doing our part to keep athletes healthy, performing at their best, and ready to get back on the field.

A great visit to Suarez Physical Therapy & Performance in Cohoes, NY, just outside Albany!

The team had seen some of my patients and wanted to learn more about ACL primary repair and BEAR approaches, especially since they still see predominantly ACL reconstructions in their area.

I had the opportunity to spend time with a fantastic group of more than 20 physical therapists, athletic trainers, strength coaches, and non-operative sports medicine physicians to share what we’ve learned about ACL preservation, patient selection, rehabilitation, and the growing options beyond reconstruction.

Always rewarding to connect with clinicians who are curious, engaged, and looking for additional ways to help their patients.

And of course, when you’re in the Albany area, you have to partake in some of the legendary local ice cream. Well worth the trip!

Regaining range of motion is one of our top priorities immediately after ACL surgery.

In those first few weeks, pain, swelling, and stiffness can make that challenging. One of the advantages of a Preservation First® approach is less disruption to the knee.

Here is one of our awesome patients just 3 days after ACL primary repair, already demonstrating 0–90° range of motion. That is a milestone we are thrilled to see this early in recovery, and something we routinely see in our ACL repair patients.

Every patient and every ACL tear is different, but for the right candidate, preserving and repairing the native ACL can offer a pathway toward getting the knee moving early and progressing through recovery.

Preserve when possible. Reconstruct when necessary.

One year out and doing great! 🙌

This patient underwent an ACL primary repair with autograft quadriceps augmentation, preserving the native ACL while adding biologic support to help reinforce the repair.

At one year, the knee is stable, strong, and back to doing what it’s supposed to do.

What a great story! One of our patients, an ER nurse from Dallas, did her homework, researched her options, and decided to fly up to NYC for a second opinion.

She turned out to be a perfect candidate and underwent an ACL Primary Repair successfully. With so few symptoms afterward, she was able to enjoy a week in New York, including a Broadway show!

Not a bad way to spend the week after ACL surgery.

Another great example of the impressive blood supply of the ACL.

The first image shows the initial view when I entered the knee. The following images show the flipped posterolateral bundle being carefully manipulated back into position.

Take a close look at those capillaries, all revved up and ready to help the ACL heal!

The final image shows the ACL after primary repair.

The patient went on to have a smooth recovery and, years later, continues to do great.

Had a fantastic weekend at the MIACH Academy in Chicago!

25 surgeons. 4 faculty. 14 states represented. A great group of surgeons coming together to learn, share ideas, and continue advancing the conversation around ACL preservation.

The Preservation First® mindset continues to grow, and we couldn’t be happier to see more surgeons embracing the idea that when appropriate, preserving the native anatomy matters.

A great weekend of education, collaboration, and momentum.
