Classic implant was removed, actually was moving already. This implant was placed around 40 years ago according to the patient, done well!!! 😎😎😎
Looks very rough surface, Sa value looks like more than 2 micro m, too rough!!! But I can see some osseointegrated bone on the surface of implant. 🙃🙃🙃
But when I saw this removed implant, implant looks crying... When you see three dots or lines, your brain recognize it as a face. We called this as "Simulacra phenomenon" in Japan, I think only in Japan, jajaja!!! 🤣🤣🤣
Anyway, big GBR procedure is coming after this removal of implant, will see!!! 😆😆😆

Posting failed implant after a long time. When i was in NYU, i saw many complications because we got many referrals from outside and somehow patients with dental implant complications came to nyu. 😳😳😳
Removal of implant is easy job if you are ready for it. But how you decide it? Usually implant got more than 50% bone loss is one of clinical decision for removal implant. But using trephine bur for removal imppant is very traumatic, better option is unscrew implant from the bone so do de-osseointegration using any instruments. Or just waiting until 100% bone loss???🤪🤪🤪
One of interesting implant removal article is applying heat generation on the failed impoant and leading bone necrosis then implant is coming out later... Hmm, way for de-osseointegration is good but patient may complain pain? Bone heal well after that? 🤔🤔🤔
So I like to use simple device, Reverse Screw Technique is my favorite! Minimum bone damage so easy for making new Tx. 😎😎😎

Sinus augmentation lecture to PG perio 2nd and 3rd year. I was thinking 2 hours for my lecture however i tuink i need 5 hours minimum...lol...so next time!!!🤣🤣🤣
Problem was lecture time, started in night time in new york and morning time in tokyo.. time difference🤔🤔🤔
Talking about history, changing thinking, material selection, and actual procedures. 😎😎😎
Today's my favorite article was, Evaluation of Maxillary Sinus Lateral Wall and Posterior Superior Alveolar Artery, A Corn Beam Computed Tomographic Study. By Danesh,-Sani SA. CIDRR 2017. In case of chronic sinusitis, the inflammatory process of the aloft tissue can create a wall thickening in 97.3% of cases with 2.6mm wall thickness on average in diseased sinuses. Interesting!!! 👍👍👍
Sinus augmentation procedure is kind of well known and established procedure. I was showing many cases and articles. Hopefully enjoying my lecture with whisky! 😆😆😆

Subepithelial connective tissue graft is still alive!!! 😆😆😆
Anyway, we can get thin one and thick one sometimes. According to Studer, the mucosa over the palatal root of the maxillary first molar was significantly thinner than at all other position in the hard palate. Kind of anatomical reason. So we should avoid this area for harvesting soft tissue from palate. Yu SK's article shows more visualized one, mesial to first molar is the safe area, and distal to first molar is the high risk area. 🤔🤔🤔

There is a chance to make a membrane perforation during antrostomy with using high speed bur, how much? Barone, Wallace, Vercelloti,,, average of membrane perforation gonna be 25% with high speed burs and 9.9% with piezo. 🤔🤔🤔
But I'm using straight diamond bur, not high speed one and membrane perforation rate during antrostomy is like 2-3%, pretty less number, almost nothing happen. This picture, membrane perforation with small sinus window was made by high speed bur. 🤧🤧🤧
Better not to have any complication so always using straight high speed but, pretty good! But if anything happen during sinus augmentation procedure, follow reenter approach published by Alex Dagoba!!! 😝😝😝
By the way, how's this motion picture? I'm huge fun of it!!! 🕺🕺🕺

So, Case of edentulous posterior mandible area, we saw many cases of thin ridge and thin soft tissue. We can place implants deeper for avoiding GBR however position of implant platform will be too deep... So try to place implants more coronally? 🤔🤔🤔

One of key information of this case is thickneaa of soft tissue. According to Berglundh, "certain minimum width of the periimplant mucosa is required, bone resorption may take place to allow a proper soft tissue attachment to from." So need certain thickness of soft tissue however this case has just 1mm thickness of soft tissue... 🤓🤓🤓

Then according to Linkevicius, "thin soft tissue thickness (less than 2mm), almost 70%/of imppants showed more than 1mm of bone loss after 1year follow up. And thick soft tissue (more than 2mm), almost 85% of the implants showed no bone loss or a loss no more than 0.5mm after 1year follow up." 😎😎😎

If you don't want to place subcreatal implant placement, you may do soft tissue augmentation. According to Wiesner, "CTG is possible with enlargement of 1.3mm might be expected." So let's do it!!! 😝😝😝

Using blade after FGG? Using high speed bur before FGG? Average thickness of epithelium layer is 0.364mm, can we control removing epithelium layer by using blade? Chance of remaining epithelium and maybe there is a risk of epithelial inclusions? Possible! But any critical problem with that? Hmm, lets see article published by Romano, IJPRD. 😎😎😎
My recommendation is taking a picture!!! Now you can see any remaining epithelium by seeing the picture. 😳😳😳

We are placing many implanta however sometime we have to remove it with some reason... 🤔🤔🤔
Because of periimplantitis? Damaged implant platform? Flowered implant platform? Easy to removed failed implant case of periimplantitis because less bone support. According to From, implant can be removed 60% cases without trephine bur. However it's difficult to remove implant case of damaged implant platform because more bone support. OS's jointed ration is oretty strong, with high SA value especially. Case of more bone supported failed implant, using trephine bur until 2/3 of implant then using reverse screw device, according to From, happen 16.4% cases. Anyway, need reversed screw device for any circumstances. 😎😎😎
Case of removal failed implant, thermal necrosis technique can be used. According to Kniha, 51℃ for 10 sec and 5℃bfor 30 sec presented significant matrix degeneration, interesting... But patient may suffer pain due to bone necrosis? 🤔🤔🤔
