18 months post stage I, 6 months post stage II.. face and neck esthetic reconstruction.

Several years ago, this beautiful young woman had an aggressive jaw tumor removed and was reconstructed with a fibula free flap (leg bone transplant) done elsewhere. She is tumor free, but had some esthetic issues that troubled her. We previously highlighted on our page after the first stage treatment with us last summer, which included repair of the tethered neck muscle which created a defect when she smiled (see prior post). A second stage treatment included removing the residual submandibular gland and fibrotic fatty tissue, volumeizing the right mandibular angle, and selectively clipping the contralateral marginal mandibular nerve to balance the smile. This last step is irreversible and we only agree to do it after a Botox trial to simulate the effect.

These are some of the most rewarding patients to care for as we can utilizing esthetic surgical techniques to reconstruct normal form and function. ❤️

We’ve selected our patient for our live surgical demonstration this June, and this is a look into her aesthetic goals in her own words.

We’re not going to share exactly what we’re planning just yet.

But I will say, it involves a new technique I’ve developed, which we are currently in the process of publishing…
and one that will in my opinion change how surgeons approach these cases. June 26-27!!

It’s not about how you make them look.. it’s about how you make the feel. Hearing people talk about how they feel after these surgeries is incredibly fulfilling. 78 going on 50. So happy for her.

This case 3 (our final) for our upcoming intensive course for .cologne in Germany this week. All of us at HFJ have become quite close to this lovely patient. Just a beautiful individual thru and thru... Couldn’t be happier for her ❤️

*The forehead bruise was from a recent incidental head bump at home.. all surgical bruising settled after a week or two.

Revision-Reduction Genioplasty, 1 year post-op. Reduction geniopalsties are complex, as are revision geniopalsties. Revision-reduction (as opposed to revision-augmentation or primary-reduction) are the hardest and most complex genioplasty types and I recommend young surgeons to ease into these kind of cases.

This is because with reduction of the chin bone, you can lose support of the soft tissue and muscles around the chin jawline neck and lower lip, profoundly aging someone. Additionally, revision geniopalsties are more complex both technically for the surgeon and emotionally for the patient, so complexity becomes compounded. In this case we were able to manage the hard and soft tissues around the chin effectively and got this beautiful young woman to a great place. Very happy for her and her particularly warm and kind family.

This one of three cases that will be presented at our upcoming intensive course with . A 34-year-old woman with early jowling and squaring of the lower face, heaviness of the cheeks, neck, and brow. There are many opinions on when to treat younger patients with a “facelift”. If a patient is in their mid 30s or early 40s, and you’re doing anything to reduce heaviness in the lower face and neck, ask yourself what that will look like in 4-5 years. in Many cases, an isolated neck lift may expose early jowling and midface-jawline laxity, even if it’s only relative to a rejuvenated neck. In these patients, treatment of the mid and lower face is often prudent to ensure long-term satisfaction and a more properly tailored result.
