If you change one thing this week, change this.

On appetite-suppressing medication you eat less of everything — and that includes the protein your body uses to protect muscle. When protein runs short, your body takes what it needs from the muscle you already have. The scale still moves. You still get smaller. But you're losing the wrong tissue, and it's the tissue that keeps your metabolism working and keeps you strong at seventy.

Roughly 100 grams a day, most days, is the target we work toward with most patients. It is genuinely hard when you're not hungry. It is also the difference between weight loss you keep and weight loss that comes back with interest.

Ask us at your next visit what your number should be — it isn't the same for everyone.

If you've heard that feeling awful is just part of it — that's not true, and believing it is why a lot of people quit in month two.

Four things that actually help:

1. Go up slower. The dose ladder is a guide, not a deadline. There is no prize for arriving early.

2. Eat before you're starving. An empty stomach makes it worse, not better. Small and regular beats large and delayed.

3. Protein first, then the rest. Smaller plate, better order, less misery.

4. Tell us early. We can adjust — dose, timing, spacing. Quitting is not the only lever, and it's the one people reach for because nobody told them the others existed.

Most of what people describe as "I couldn't tolerate it" was a dose and timing problem nobody fixed.

Just about every clinic in this business posts before-and-after photos. We're not going to.

Not because we don't have results worth showing. Because the person in that photo is a patient, and turning a patient into an advertisement changes something we'd rather leave alone. Even with permission — and it would always be with permission — it sets up a comparison you never asked for, against a stranger whose starting point, thyroid, medications, sleep and life you know nothing about.

It also teaches the wrong lesson quietly: that the goal is to look like somebody else's photo. It isn't. The goal is your labs, your strength, your energy, and your knees at sixty.

So you'll get plain talk and real numbers from us instead. If that makes us worse at marketing, we can live with it.

The weight stops moving. It happens to almost everyone, usually somewhere around month three, and it's the point where most people quietly give up.

Here's what we'd tell you if you were sitting in front of us.

A stall is not a failure — it's information. Your body has adapted, which is what bodies do. It usually means something needs adjusting: the dose, the protein, the sleep, the strength work, sometimes the medication itself. Occasionally it means a thyroid or a hormone is doing something worth looking at.

What it almost never means is that you did something wrong, or that you're one of the people this doesn't work for.

If you've been stuck for more than a few weeks, don't wait it out alone. That's the visit we're for.

Nobody ever asks to see the fridge. You should.

Every vial we dispense lives in a locked, pharmacy-grade refrigerator held between 2 and 8 degrees Celsius, with an alarm that trips if it drifts and a temperature written down and initialed every business day. That's 4.0 on the display, top right — right where it belongs.

This is the unglamorous half of the job, and it's the half nobody markets. It's also the half that decides whether what you're paying for actually works. A vial that got warm looks exactly like a vial that didn't.

The scale is the least useful tool in our building. We own three of them.

Here's the problem with it. It gives you one number for a body made of muscle, fat, bone and water — and it will not tell you which one moved. Drop four pounds of muscle and it congratulates you. Add two pounds of muscle while losing three of fat and it acts like you wasted a month.

So we don't make decisions off it by itself. We look at what's actually changing, and whether the weight coming off is the weight you wanted off. That distinction is most of the difference between losing weight and getting healthier.

Something you won't hear in most ads: these medications genuinely work. That part is real.

What's usually missing is everything around them. Whether anyone checked the thyroid. Whether muscle is being protected. What happens when someone stops.

The medication is one piece. The program around it decides whether the result lasts.

That's the part we build.

Before anyone here starts semaglutide or tirzepatide, we draw blood.

Metabolic panel. Thyroid. Hormones. Inflammatory markers.

That isn't standard. Plenty of programs will start someone on a GLP-1 without ever knowing what their thyroid is doing.

We'd rather spend the first visit finding out than spend six months wondering.

The paperwork is the hard part. We handle it.
