This patient has been self-conscious about his skin since he was fifteen. By sixteen or seventeen, the acne was severe enough that despite seeing an esthetician, scarring was inevitable.
During his consultation, I examined his cheeks under angled light. The right side shows more significant fat destruction in a critical structural zone, an area that traditionally supports the face. Over time, this loss causes surrounding tissue to shift and pull down. The scars are tethered, which means the skin is physically anchored beneath the surface. The surrounding tissue compensates by thickening, creating an uneven contour.
He had three sessions of profractional laser elsewhere, spending four thousand dollars with no manual techniques like subcision to address the tethering. Laser alone cannot release what is anchored down.
His case is manageable and solvable. We will release the fibrous bands, smooth the thickened edges, and restore the structural support that was lost. His cheekbones and overall facial volume are excellent. The problem is isolated to the scars, and that is exactly what we address.
