This patient has been self-conscious about his skin since he was fifteen. By sixteen or seventeen, the acne was severe enough that despite seeing an esthetician, scarring was inevitable.

During his consultation, I examined his cheeks under angled light. The right side shows more significant fat destruction in a critical structural zone, an area that traditionally supports the face. Over time, this loss causes surrounding tissue to shift and pull down. The scars are tethered, which means the skin is physically anchored beneath the surface. The surrounding tissue compensates by thickening, creating an uneven contour.

He had three sessions of profractional laser elsewhere, spending four thousand dollars with no manual techniques like subcision to address the tethering. Laser alone cannot release what is anchored down.

His case is manageable and solvable. We will release the fibrous bands, smooth the thickened edges, and restore the structural support that was lost. His cheekbones and overall facial volume are excellent. The problem is isolated to the scars, and that is exactly what we address.

A question from my live stream: why do some patients feel their scars look worse after multiple microneedling sessions?

The answer lies in the mechanical motion itself. Microneedling involves repeated friction and rolling across the skin surface. For certain scar types, particularly boxcar scars, this motion can actually stretch and widen the indentation rather than filling it. The scar edges get pulled and the base spreads, making the defect more visible over time instead of less.

This is not a universal outcome. Some patients see temporary improvement from the microswelling and collagen stimulation. But for scars with already compromised structural support, the mechanical trauma can exacerbate the problem. The needles do not reach the depth where tethered bands reside, so the root cause remains unaddressed while the surface gets manipulated repeatedly.

That is why I emphasize understanding scar architecture before choosing a modality. What works for fine lines and skin quality does not automatically translate to indented, atrophic scarring.

This Fitzpatrick 4 patient presented with moderate to severe boxcar scars across the cheeks and temples. These are the consultation photos she submitted, using angled lighting to reveal the true depth and shadows of the scars.

The treatment plan required a multimodal approach. We began with TCA Cross, applying our custom blend to each scar which will rebuild collagen from the base upward. Tumescent solution raised the skin and scars away from deeper structures, creating a cushioned plane that made subcision safer and more effective by releasing thick scar tissue.

For scars too deeply tethered to respond to lifting alone, micro punch excision removed the damaged tissue entirely, converting bound-down divots into narrow defects that close as fine lines. Pinpoint erbium laser ablated thick, raised edges with sub-millimeter precision, blending borders without thermal damage to surrounding tissue. We placed Bellafill strategically after subcision, using 80% bovine collagen for immediate support and 20% PMMA microspheres as a permanent scaffold for ongoing collagen production in areas of fat loss.

We finished with full field erbium resurfacing to treat the entire facial canvas uniformly, ensuring seamless integration between treated scars and healthy skin for cohesive, natural healing.

Back-to-back procedures. Complex cases. Patients who had been waiting years. New York City was a trip to remember.

PIE and PIH marks are two of the most common post-acne concerns I get asked about. The good news is there are simple, over-the-counter options to start with at home.

Azelaic acid is my first recommendation. It helps calm inflammation and fade discoloration without being overly harsh. I suggest starting with 10% azelaic acid in the morning. At night, Eucerin pigmentation cream is a solid option for continued brightening and evening out tone.

The key with both is consistency and sun protection. Without daily sunblock, these products will struggle to keep up with new pigment production triggered by UV exposure.

This is a safe, accessible starting point before considering lasers or prescription topicals. Give it time. Three months of consistent use is typically when patients start noticing visible improvement.

This patient traveled from Arizona to our Newport Beach office to begin his scar restoration journey. He presented with severe acne scarring concentrated in the upper region of his face, including deep tethering across the cheeks and forehead that required a comprehensive first-phase approach.

We began with a custom TCA Cross session to target the deepest ice pick and boxcar scars, placing acid at the precise depth needed to stimulate collagen rebuilding from the base upward. Multi-modality subcision followed, using specialized needles to release fibrous bands in both the cheeks and forehead. Forehead subcision demands particular precision given the thin tissue and underlying anatomy, but it is essential for addressing scars that pull the brow and glabellar region down.

We placed Sculptra to begin restoring volume lost to inflammatory fat destruction, taking advantage of the wound healing response triggered by subcision to optimize collagen stimulation. Finally, full-face Erbium resurfacing ensured seamless blending across the entire treatment canvas, smoothing texture and promoting uniformed healing.

This was phase one. We will monitor his collagen response over the coming months before determining the timing and composition of phase two.
