Aging Waiver: Aging Home and Community-Based Waiver Services may be available to Pennsylvanians over the age of 60 to enable them to continue to live i

Attendant Care/Act 150 Waiver: If you have a physical disability, the Attendant Care Waiver and state funded Act 150 program may be available to you to continue to live in your home and community with support and services. Obra Waiver: The OBRA Waiver is a Home and Community Based Waiver program that may help you if you have a developmental physical disability to allow you to live in the community and remain as independent as possible. Commcare Waiver: A Home and Community-Based Services Waiver for Individuals with Traumatic Brain Injury (TBI). We are available on phone 24/7. You can call us anytime of the day, any day of the week. Our Senior Care representative can help you decide if our services could assist your loved one in preserving a good quality of life and remaining safely in his or her home. When you call River Oaks Home Care, a representative will set a time to visit you with a no-obligation personal consultation. We have hourly and around-the-clock care. Services Include:

This year marks ten years of River Oaks Home Care serving seniors and families throughout the Philadelphia region. Over the past decade, we have had the chance to walk alongside families through caregiving changes, new diagnoses, recovery periods, long nights, and everyday routines that help people remain comfortable at home.

We have also been honored with recognition including Philadelphia100, Inc. 5000, Employer of Choice, Provider of Choice, Leader in Experience, and more.

What we appreciate most, though, are the relationships built over time with families, caregivers, referral partners, and the communities we serve.

One fall does not automatically mean a loved one needs 24-hour care. But repeated falls, trouble getting up safely, growing confusion, or needing more help throughout the day can signal deeper safety concerns at home.

Many seniors living with dementia experience increased confusion, restlessness, or disrupted sleep after sunset, a pattern often called “sundowning.”

Changes in lighting, fatigue, and shifts in the body’s internal clock can all make evenings more challenging.

Keeping a consistent evening routine, reducing noise and stimulation, and using soft lighting can help create a calmer environment at night.

If overnight confusion or wandering becomes more frequent, it may be a sign that additional care support or routine adjustments are needed to keep your loved one safe and comfortable.

Denitra Hedgepeth approaches caregiving with a simple goal each day, helping someone feel cared for and helping them smile.

As a Home Health Aide, she values the opportunity to make a real difference in someone’s daily life through patience, conversation, and dependable support. The work may look different from home to home, but for Denitra, the heart of caregiving stays the same. People want to feel comfortable, respected, and understood.

Outside of caregiving, she enjoys spending time with loved ones and finding moments to relax and recharge, something she believes helps her stay present for the families she supports.

Caregivers are often the first people to notice changes in sleep, mobility, confusion, or safety concerns of their loved ones, but often fail to see changes within themselves. They are also often the last people to ask for help.

Respite care is not stepping away from responsibility. It is creating space to rest, regroup, attend appointments, or simply recover from the pace of caregiving.

Even short periods of support can help families think more clearly about what level of care may be needed moving forward.
