"My clothes don't fit and I freaked."

Nobody tells you this: in midlife your body does not just change how MUCH it stores. It changes WHERE. There is a name for it, and Dr. Warren explains it in 30 seconds.

Drop a YES if your waistband knows exactly what she is talking about. Full guide: link in bio.

She was 53. Still running three days a week. Still eating the same careful meals she had eaten for a decade. And she had gained 11 pounds in 14 months without changing a single habit.

She sat in my office convinced something was wrong with her. Something WAS happening, but it was not a character flaw. It was the estrogen drop shifting her fat storage. It was muscle loss quietly lowering her daily burn. It was disrupted sleep pushing cortisol and cravings in the wrong direction.

When I explained it, she did not feel defeated. She felt relieved. Because you cannot fix a problem nobody has ever correctly described to you.

That explanation, and the plan that goes with it, is now a 49-page guide. Link in bio.

"I'm doing everything right and the scale won't move."

Dr. Warren hears this sentence in her practice every single week. And her answer surprises most women: you probably ARE doing everything right. For a body you no longer live in.

Watch to the end for what actually changed, and what works now.

Have you ever said this exact sentence? Tell me below. Full guide: link in bio.

The diet that worked when you were 35 was designed for a body you no longer live in.

That body ran on steady estrogen, solid muscle mass, and calm cortisol. It could handle a hard calorie cut. It could cardio its way out of a heavy month.

The perimenopause body is a different biological environment. Cut calories hard now, especially without enough protein, and your body burns muscle along with fat. Less muscle means a slower metabolism, which means the harder you push the old way, the more stuck you feel.

You didn't fail. The rules changed. And when you learn the new rules, things start to move again.

That is exactly what my new guide teaches. Link in bio.

What is one strategy that used to work for you and just stopped? Tell me below. Your comment might be next week's post.

"I weigh the same, but nothing fits."

I hear this sentence in my practice almost every week. And the woman saying it usually thinks she is going crazy, because the scale agrees with her and her waistband does not.

She is not going crazy. Here is what is actually happening: when estrogen declines, your body changes WHERE it stores fat. It shifts away from hips and thighs and toward the abdomen, including the deep visceral fat around your organs. Your weight can hold perfectly steady while your shape reorganizes itself.

That is not a willpower problem. It is a hormone story, and once you understand it, you can actually do something about it.

I wrote a full guide explaining exactly this, plus what works now. Link in bio.

If you have gained weight in perimenopause or menopause and you are blaming yourself, I need you to hear this: you did not get lazy. Your discipline did not disappear. Your biology changed.

Estrogen dropped, and with it went the rules you spent 30 years learning. Fat storage moved. Muscle started slipping away quietly. Your resting metabolism followed it down. The old playbook stopped matching the game.

I have spent 30 years in medicine, 23 of them in obesity medicine, having this exact conversation with women in my practice. I finally wrote it all down in one plain-language guide. It is called "Menopause Weight Gain Is Not Your Fault," because those are the words I say most often, and the words most women have never once heard from a doctor.

The full guide is linked in my bio. Instant download, written the way I talk with my patients.

Tell me in the comments: has a doctor ever actually explained this to you? I read every reply.

The FDA just updated how obesity treatment is approached — and it's a meaningful shift.

Treatment decisions are no longer based on BMI alone. Doctors are now looking at the full picture: cardiometabolic risk, existing health conditions, and what's right for each individual patient.

Current medications are showing real results — not just significant weight loss, but measurable improvements in heart health and overall wellbeing. And they're being introduced earlier, rather than as a last resort.

This is what evidence-based obesity care looks like in 2026.

At Physicians Healthy Weight Center, this has always been our approach — treating obesity as the chronic medical condition it is, with the individualized attention it deserves.

If you've been waiting for the right time to get serious about your weight and health, the time is now. Feel free to reach out to our office — we'd love to have a real conversation with Dr. Warren about where you are and where you want to be.
