Ready to Return Home

When Gioia Olson arrived at Oconto Health & Rehabilitation, she was unable to complete the stairs needed to return to her second-level apartment and required moderate assistance with daily activities.

With determination and the support of our therapy team, Gioia made incredible progress. After one month, she met 100% of her therapy goals. At discharge, she was able to complete stairs independently and perform all of her ADL tasks on her own, allowing her to safely return to her apartment.

“Everyone I worked with is very nice. The therapy department is awesome. They made me push myself to reach my goals. I am feeling very good about returning home this time and being successful.”

We are proud of Gioia’s hard work and wish her continued success back at home.

Oconto Brings Back Independence

Connie Scofield came to Oconto Health & Rehabilitation requiring maximum assistance with mobility and daily activities due to decreased strength. With determination and the support of our therapy team, Connie made excellent progress throughout her stay.

By discharge, Connie was performing pivot transfers and bed mobility with standby assistance and completing her daily activities with setup to supervision. After meeting 100% of her therapy goals, she was able to return to her assisted living facility.

“My therapy went well! I am back to being independent, and my strength is back to where it was before. I am happy to return to my ALF. The facility provided good care. I would return to this nursing home in the future if needed.”
