The most useful part of a shoulder appointment happens before any treatment does.

"Shoulder pain" names a location, not a problem. Underneath it sit several different things — irritated rotator cuff tendons, pain in the subacromial space, a stiffening capsule, arthritis, and pain that's generated in the neck and simply felt in the shoulder. They need different plans.

So we ask questions that sound oddly specific, and each one is doing a job.

"When did it start, and what changed around then?" Tendon problems usually follow a change in load — a new job, a new gym programme, a weekend painting a ceiling.

"Which movements bring it on?" Pain in a particular arc of overhead reaching points somewhere different from pain that's there in every position.

"Does it wake you at night?" Being unable to lie on that side is a recognisable pattern — and it's the thing that most reliably wears people down.

"Does it travel past your elbow? Any pins and needles?" That's the neck question. A shoulder whose pain comes from the neck won't improve with treatment aimed at the shoulder, and we'd rather know on day one than in week six.

"What do you most want back?" We write it down, because that's the outcome the plan is actually for.

Where the picture is a rotator cuff or subacromial problem, we have decent randomized evidence to work from — 17 trials, 20.41mm of pain relief over placebo on its own and 16.00mm alongside exercise, on a 100mm scale. That's an adjunct with modest, real effects: it works alongside exercise and rehabilitation rather than replacing them.

And sometimes the assessment leads us to say we're not the right people, or that this shoulder needs imaging or a surgical opinion first. That's a good outcome for an appointment, not a wasted one.

What actually happens during a course of treatment for shoulder pain? Here is the whole sequence, start to finish.

Visit one is mostly assessment. History first — when it started, what changed in your activity around then, what specifically provokes it, whether it wakes you at night. Then examination: range of motion, strength through specific patterns, and the tests that separate a rotator cuff problem from subacromial pain, a stiff capsule, or pain that's actually coming from the neck.

Then we write down where we're starting. Pain scores at rest and on the movements that hurt. Range of motion in degrees. A short validated function questionnaire. And the one thing you most want back — sleeping on that side, the top shelf, a round of golf.

We set and record the treatment parameters too: wavelength, power, energy per point, number of points, and exactly where each one was placed. There's a reason for that bookkeeping. In those 17 shoulder trials, adequately dosed treatment produced clinically important pain relief — 20.41mm over placebo on its own, 16.00mm alongside exercise — and the benefit tracked closely with whether the dose was adequate. The same pattern shows up in the knee literature: 18.71mm for doses following international dosing recommendations, against 6.34mm for doses outside them.

A usual course runs two to three sessions a week for four to six weeks, then a review where we re-measure everything and compare it against the starting numbers. If the numbers aren't moving, that's information — it should change the plan, not get absorbed into a longer package.

And the part that does the heavy lifting: this is an adjunct with modest, real effects. It works alongside exercise and rehabilitation rather than replacing them.
