Here's what we check for at each visit:

✅ Microalbumin: an early warning sign of kidney damage
✅ Glucose: reflects your blood sugar levels
✅ Microprotein: can indicate kidney filtering issues or inflammation
✅ Creatinine: a waste product that shows how well your kidneys are filtering
✅ Hemoglobin: may signal infection, kidney stones, or other urinary issues
✅ Nitrite & Leukocyte Esterase: help screen for UTIs and inflammation
✅ pH, Specific Gravity, Bilirubin, Urobilinogen, and Beta-Hydroxybutyrate: give a full picture of hydration, liver function, and metabolic health

Did you know KSCO offers both Remote Patient Monitoring and Chronic Care Management for our patients? Through our partnership with Tellihealth, we help keep you connected to your care team between visits. Here's how each program works...

👉 Remote Patient Monitoring:
• You'll receive a blood pressure monitor connected to a secure platform
• Your readings are sent automatically to your KSCO clinical team
• Tellihealth monitors your readings in real time
• If your blood pressure is too high or too low, you and your provider are contacted right away for early intervention

👉 Chronic Care Management
• You'll be assigned a dedicated care coordinator who gets to know your health goals and history
• Your care team creates and maintains a personalized care plan tailored to your conditions
• Proactive outreach keeps you on track with medications, follow-ups, and preventive care
• Your KSCO providers stay informed in real time for faster responses to changes in your health

Both programs are available at little or no cost for many patients, depending on your insurance plan, and may be covered by Medicare.
