"My insulin is still high β is my GLP not working?"
A patient on today's call is doing beautifully β down in weight, feeling great, energy back. But she was worried.
"I feel amazing, but my labs with my primary care physician still show high insulin. Is my medication not working? Do I need something additional?"
I smiled and told her: relax β this is more common than you'd think, and it doesn't mean anything's wrong.
Here's what I explained:
Semaglutide actually lowers fasting insulin β by about 20% in clinical trials. So when insulin is still elevated, it's not a sign of drug failure. It just means we need the full picture, not one number in isolation.
Our next steps:
β Pair insulin with glucose (HOMA-IR) instead of reading it alone
β Confirm the draw was a true fast
β Re-check in 6 months rather than reacting to one number (this is for re-assurance, I donβt actually check these levels routinely because it likely wonβt change my management)
β Layer in lifestyle levers that directly improve insulin sensitivity: strength training, cardiovascular exercise, more whole foods, less processed carbs and less saturated fat
