HOUSE CALL NOTES: The Fragile First Week After the Hospital

Coming home from the hospital is a relief—but the first several days at home can be an important part of recovery.

There may be new medications to understand, discharge instructions to follow, wounds to monitor, and changes in symptoms that families aren't sure how to evaluate.

A post-discharge house call gives the patient an opportunity to be evaluated where recovery is actually happening: at home.

During a transitional care visit, our medical team can review medications, assess recovery, check for potential concerns, and help make sure the care plan is moving in the right direction.

For families preparing for a loved one to come home from the hospital, planning follow-up care before discharge can make the transition much smoother.

Fall health starts before fall arrives.

Late August is a good time to prepare for respiratory illness season—especially for older adults and people managing heart disease, lung disease, diabetes, or a weakened immune system.

Put these three questions on the calendar:

When should the annual flu vaccine be scheduled?
Is an RSV vaccine recommended based on age and health risks—and has it already been received?
What COVID-19 vaccine plan is appropriate based on age, medical history, and previous doses?

For most adults, the CDC recommends scheduling the flu vaccine during September or October. RSV and COVID-19 recommendations are more individualized, so review them with a physician or pharmacist.

A little planning now can prevent confusion once respiratory season is underway.

Yes—a house call doctor can become a patient’s primary care provider when the practice offers ongoing care, not simply one-time visits.

That means routine follow-ups, medication review, chronic-condition monitoring, preventive planning, test coordination, and communication with specialists—all while the patient remains at home.

Our new article explains what families should confirm before switching, how Medicare coverage may work, and which services may still require an outside facility.

HOUSE CALL NOTES, — Five health facts every family caregiver should keep handy:

1. Usual blood pressure range
2. Most recent A1C, if diabetic
3. Number of daily medications
4. Date of the last hospital or ER visit
5. Baseline weight

Keep these details on one card—and save a photo of it on your phone. Update it after appointments, medication changes, or hospital visits.

Why does it matter? When something changes, having the right information can turn a stressful phone call into a more productive conversation with the care team.

A sudden weight change may deserve attention. For people with heart failure, follow the weight-monitoring and reporting instructions provided by their healthcare professional.

If your parent lives in assisted living, one question matters more than most families realize: What happens when they need a doctor?

A routine appointment can require transportation, time away from work, mobility assistance, and hours outside the community. Mobile physician care changes the logistics by bringing scheduled medical visits directly to eligible residents where they live.

Our new article explains how assisted living partnerships work - from resident intake and on-site visits to staff communication, family updates, testing coordination, and follow-up care.

Horizon Mobile Physician Services serves eligible patients in Palm Beach County and select communities in Martin and St. Lucie Counties. Medicare accepted for eligible services. Appointment availability varies.

A HEALTH NUMBER WORTH KNOWING:

The CDC estimates that 34% of U.S. adults age 65 and older have chronic kidney disease.

Even more surprising: 87% of adults with chronic kidney disease do not know they have it.

Early kidney disease often causes no symptoms. For older adults with diabetes or high blood pressure, blood and urine tests can help detect changes before the disease becomes advanced.

During a scheduled home visit, our providers can review risk factors, monitor blood pressure and determine whether kidney-function testing is appropriate.

An office fall-risk assessment checks how someone walks on a smooth clinic floor, under bright lights, in the shoes chosen for the appointment.

Falls usually happen somewhere very different.

They happen on the hallway rug. On wet bathroom tile. Along the dark path to the bathroom at 2 a.m.

An in-home assessment allows a medical provider to evaluate the complete picture: gait on actual floors, lighting, rugs, bathroom setup, footwear, mobility equipment, medications and dizziness.

Our new article explains what a doctor looks for—and why a fall risk assessment should happen where the risk actually lives.
