Medicare has proposed a new rule that would penalize doctors for performing necessary procedures (like biopsies, freezing or skin cancer removals) on the same day as an office visit or skin exam.

This rule would affect procedures for all of your doctors, not just dermatologists. Cutting payments by 50% is unsustainable and many practices have a choice, go out of business or schedule 2 separate appointments instead of one.

What this means for you:
1. Two Copays
2. Two Trips
3. Two Days Off
4. Weeks of Waiting
5. Delayed Diagnosis

Urge Congress to Stop Medicare Cuts That Will Turn One Visit Into Two Visits. **September 14, 2026 Deadline to Comment**

A little-noticed provision buried in the proposed CY 2027 Medicare Physician Fee Schedule (CMS-1848-P).

Payment for an office visit will be cut by 50% when a minor procedure — a biopsy, a joint injection, a lesion removal, an incision and drainage — is performed on the same day.

And this will not stop with Medicare.
Private insurers routinely mirror Medicare payment policy. Whatever CMS finalizes for seniors will almost certainly be adopted by commercial plans soon after, affecting Working-age Adults, Families, and Children.

It will fundamentally change how patients experience care.

The likely result: two appointments instead of one

When Medicare penalizes practices for same-day care, practices will be forced to schedule the evaluation on one day and the procedure on another just to stay financially viable.

For patients — especially older adults, people with mobility limitations, rural patients, and anyone who depends on a caregiver for transportation — that is not a small inconvenience.

It means:
• Another copay and more out-of-pocket cost
• Another day off work for the family member who drives them
• Another round of transportation, parking, and waiting
• More days of anxiety between finding a suspicious lesion and getting it biopsied

Delay is not clinically neutral

Same-day biopsies catch skin cancers early. Same-day drainage prevents infections from worsening. Same-day injections relieve pain that would otherwise linger for weeks.

Turning routine one-visit care into routine two-visit care will cost patients time, money, and — in many cases — better outcomes.
