🌟 Milestone to Mobility 🌟

Today we are celebrating an incredible success story!

When Jelp “JP” Barber came to Community Health & Rehabilitation for short-term rehabilitation, he had one clear goal: to regain his independence and be able to walk the length of his dock so he could return to the life he loves. As the owner of an oyster farm, every step forward meant one step closer to getting back on the water.

Through determination, hard work, and the support of our dedicated rehabilitation team, JP has achieved his first milestone and is discharging home with renewed independence. Even more exciting, he is already making great progress toward his long-term goal of independently managing his oyster farm once again.

Stories like JP’s remind us that rehabilitation is about so much more than recovery. It’s about restoring confidence, independence, and the ability to return to the people, places, and passions that make life meaningful.

Congratulations, JP! Thank you for allowing Community Health & Rehabilitation to be part of your journey. We wish you continued success as you return to the dock and continue building toward your goals.

CNA Week officially kicked off last Thursday, and we have been celebrating our incredible Certified Nursing Assistants each day for the compassion, dedication, and exceptional care they provide to our residents.

We are especially grateful to Peoples Hospice for recognizing our CNA team with a wonderful snack bar and for helping us highlight the vital role CNAs play in the health, comfort, and well-being of our patients. Their hard work, kindness, and commitment make a lasting impact every single day.

Thank you to our amazing CNAs for all that you do, and thank you to our community partners for helping us celebrate these healthcare heroes!

In collaboration with our community post-acute care partners, Community Health & Rehab is proud to host Beyond the Bedside, a monthly coffee and networking event designed to strengthen collaboration across our healthcare community.

Each month will feature a spotlight session led by community partners, focusing on meaningful discussions surrounding patient needs, care coordination, and opportunities to better serve those in our community through shared education and resources.

Together, we can continue building stronger connections and improving outcomes for the patients and families we serve. We look forward to learning, networking, and growing alongside our valued partners. 🤝

A huge THANK YOU to everyone who came out to our Pizza Bar Meet & Greet last week! Even with the pouring rain, the turnout and support were incredible, and we truly felt the love from our community and fellow healthcare partners. 💙
We had such a great time connecting, sharing laughs, and celebrating new beginnings as we officially welcomed our new Director, Aleashia Wood, and our new Director of Marketing, Stephanie Worley! 🎉
To all the agencies, friends, and community members who braved the weather to be there — thank you for making the evening so special. Your support means the world to us, and we’re excited for all that’s ahead!
