US Air Force Medical Service: Synensys has partnered with the Air Force to provide Patient Safety Specialist programming for 10+ years resulting in a 70% reduction in preventable patient harm events. Many of the 70+ medical treatment facilities have reduced serious safety events to zero as a result of the full implementation of system safety practices and high reliability principles. US Air Force Air Mobility Command: Air Mobility Command has been able to develop a customized Line Operations Safety Audit (LOSA) program capable of interfacing and integrating with multiple safety management data sets leading to comprehensive system improvement in safety culture, safety reporting, and threat and error management performance. Synensys successfully implemented new LOSA programming with Aeromedical Evacuation, Aviation Maintenance, and the Air Operations Center command

WellSpan Health: WellSpan developed a comprehensive system safety program for their largest hospital perioperative service line. Synensys conducted an organizational assessment of team-based safety practices, integrated change management tools and HRO principles, provided safety training for senior leaders, frontline trainers, and coaches, developed a system safety dashboard, and configured technologies to manage operational risk, improved quality of care, and increased patient safety. 100% of surgeons, nurses, residents, technicians, and anesthesia providers were trained despite COVID-19 surge operations.

🚨 A hospital was recently placed in "immediate jeopardy" after a series of patient safety failures involving monitoring breakdowns, delayed responses to telemetry alerts, patient misidentification, and infection-control gaps.

The easy explanation is to focus on individual mistakes.

The harder—and more valuable—question is:
❓ What system defects made these outcomes possible?

Traditional investigations often focus on what failed. Systems-based approaches like CAST (Causal Analysis based on Systems Theory) encourage us to examine the controls, feedback loops, communication pathways, staffing models, and governance structures that shape everyday work.

When similar failures occur across multiple units, technologies, and processes, the issue may be larger than any one person or event.

Safety improvements do not fail because organizations stop caring.
They fail because the system was never designed to sustain them.

Policies get updated. Training gets delivered. Teams respond when issues happen. But over time, competing priorities, inconsistent follow-through and communication gaps can quietly pull organizations back into reactive cycles.

That is why lasting safety is not built through isolated actions.

It is built through systems that make learning, accountability and improvement part of everyday operations.
As part of National Safety Month, Synensys is highlighting what helps safety improvements last over time, not just immediately after an incident or initiative.

In one anonymized organization, safety efforts were largely reactive. Teams worked hard to respond after issues occurred, but the organization lacked consistent mechanisms to support continuous learning, follow-through and prevention across the system.

The commitment was there. The structure to sustain it was not.

Our approach: Strengthen the system, not just the response
