The new Centennial Healthcare & Rehabilitation Center, is conveniently located near Fairmount Park in West Philadelphia with direct access to public transportation. Centennial is a beautifully renovated facility with many dayrooms and a large dining area. Our residents enjoy the comfortable, safe and cheerful environment at Centennial and can choose from the many activities or events ongoing on a

daily basis. With added emphasis on quality and compassion, you can be assured of peace of mind that your loved one is receiving quality care you can trust. Centennial has its very own Concierge Department available to tend to any specific needs or concerns a resident may have.

Rehab Success Spotlight: Mr. Hutton ❤️

When Mr. Hutton arrived at Centennial, he was only able to walk short distances and needed support with his mobility.

Through determination, hard work, and the support of our rehabilitation team, he made incredible progress. Today, he's walking independently and ready to return home. 🏋️

💦 Wheelchair Wash Day was a whole vibe! 😎🌈

What started as a simple way to keep wheelchairs and walkers fresh turned into an afternoon full of smiles, laughter, and summer fun.

Our team got to work with the soap, the rainbow wash tunnel, and plenty of helping hands to make sure every chair got the full treatment. Residents and staff enjoyed the sunshine, the laughs, and the chance to make a routine task a little more fun.

Clean chairs, cool water, and memories made along the way. That’s what summer is all about! ☀️
