Centennial Healthcare and Rehabilitation Center Providing skilled nursing, rehabilitation, and long-term care with specialized cardiac, orthopedic, respiratory, and wound care in the heart of Philadelphia.

The new Centennial Healthcare & Rehabilitation Center, is conveniently located near Fairmount Park in West Philadelphia with direct access to public transportation. Centennial is a beautifully renovated facility with many dayrooms and a large dining area. Our residents enjoy the comfortable, safe and cheerful environment at Centennial and can choose from the many activities or events ongoing on a daily basis. With added emphasis on quality and compassion, you can be assured of peace of mind that your loved one is receiving quality care you can trust. Centennial has its very own Concierge Department available to tend to any specific needs or concerns a resident may have.

Pink for a Purpose. 💗

Our Centennial team came together for Breast Cancer Awareness Month by wearing pink and creating a wall filled with messages of support.

Team members filled out cards with words of encouragement, honored breast cancer survivors, and shared messages for those currently fighting. They then added their cards to our display as a reminder that no one has to face it alone.

We also handed out pink pins, bracelets, keychains, candy, tissues, and hand cream to keep the support going throughout the facility.

A little pink, a lot of heart, and plenty of reasons to keep raising awareness. 💗

🎀 Breast Cancer Awareness Month 🎀

October is Breast Cancer Awareness Month - a time to raise awareness, support those affected, and remind one another of the importance of taking care of our health.

We stand with breast cancer survivors, those currently fighting, and the families and loved ones who support them. Join us on October 5th as we distribute bracelets, candy, and inspirational cards to show our support and help spread awareness. And don't forget to wear pink!

Together, we can encourage conversations about breast health and the importance of early detection. 🎀

🎉❤️ Another Restore Strength Rehab Success Story! ❤️🎉

We’re proud to celebrate Mr. Brown and the progress he made during his time at Centennial! After coming to us following recurrent falls at home, Mr. Brown underwent spinal surgery and returned to Centennial to continue his rehabilitation.

Through hard work and consistent therapy, he made incredible progress with his balance and coordination, mastering both walking and stair climbing. 💪👏

Mr. Brown shared, “I'm grateful for everything you all have done for me and I will be back to visit.” ❤️

We’re so happy to see Mr. Brown reach this milestone and wish him all the best as he continues his journey! 🏡✨
