In , Khela & Gendelberg determine that while the AO upper cervical injury classification system provides a much needed framework for classifying traumatic injuries from the occiput to the C2-C3 junction, it should not be relied upon for clinical decision-making or research purposes due to low interobserver reliability.

"In particular, the moderate agreement for injury type, especially for ligamentous (Type B) injuries, results in a risk of disparate injury classification and inconsistent treatment strategies, undermining the system’s suitability for standardized use," the authors write.

🚩 Free to read until June 14: "One might surmise that if a patient threatens to harm another person over the course of his or her treatment, this would trigger a clear-cut set of duties on the part of the treating physician. After all, in the 1970s, the California Supreme Court sought to balance public safety with patient confidentiality in a landmark ruling, which held that the duty to protect the public can override patient-physician confidentiality," writes B. Sonny Bal MD, JD, MBA, PhD in his latest column. "But, as later case law shows, the application of this duty is not as straightforward as one might expect."

🔸All were cleared through the 510(k) pathway, with most (62%) claiming substantial equivalence to non-AI predicate devices

🔹Only 11 (30%) were evaluated in peer-reviewed studies—the majority (67%) of which were affiliated with manufacturers

🔸Only 8 (22%) explicitly stated that their AI algorithms were fixed, static, or nonadaptive

🔹14 (38%) were also approved for use in the EU

"These findings support the need for clearer reporting standards and targeted regulatory refinements to ensure that AI-specific risks are appropriately assessed within existing oversight frameworks, particularly in the domain of selecting technologically appropriate predicates," the authors write.

"It may be that future readers will look back at the issues raised by Bracken et al. and think, 'how quaint!'" writes Paul A. Manner MD in the Editor's Spotlight/Take 5 spotlight on this study. "But until that day, clinicians should approach AI as they might approach any other novel treatment: with caution, and with the understanding that just because a device has been approved does not mean it needs to be embraced."

"Fortunately, incorporating practical concepts from Bayesian reasoning into our decision-making framework may help us overcome the common errors that contribute to these problems," they write. "The Bayesian framework is not a parlor trick for statisticians; it is simply the formalized version of the same iterative, context-sensitive reasoning surgeons have always employed."
