Hello everyone, I'm here again and happy to share a beautiful long D/C Success journey at EF (with some help from MS)– Deb Brownback D/C to home with Family after almost 2-year rehab stay.

"Deb Brownback initially admitted to AristaCare Mearings in January 2024 with dx of respiratory failure. At that time, she was totally dependent upon staff for all aspects of functional mobility and ADLs. Deb received all medication and nutrition via PEG tube and was unable to eat or drink food by mouth. Additionally, Deb required mechanical ventilation via tracheostomy to meet her respiratory needs. Throughout her stay, Deb was diagnosed with end stage renal disease and would require dialysis. Deb was transferred to AristaCare East Falls to continue her rehabilitation while being able to receive inpatient dialysis at East Falls. It was there that she really started to excel in her rehabilitation and significant progress was made. Respiratory therapy was able to completely wean Deb off the ventilator, and progress to decannulation, no longer requiring supplemental O2 to meet her needs. Deb showed great progress during her work with speech therapy, she is able to hold conversational speech, demonstrate improved cognitive strategies, and progressed to a regular diet with thin liquids and no longer at risk for aspiration. Deb worked diligently with PT and OT services, and at time of discharge to home, Deb is able to perform bed mobility with independence, transfers from bed/chair and to bedside commode with Minimal assistance, able to perform upper body ADLS with set up assist, and lower body ADLS with moderate level assistance. PT was also able to facilitate improved dynamic balance and significantly reduce risk for falls during functional tasks. Deb is ambulating short household distance with rolling walker and is able to ascend/descend 3 steps with assistance to be able to enter/exit her home. It was a very long journey, with some ups and downs, but Deb was determined to put in the work without team to reach her goal of discharging back to home with family! We would like to highlight a huge success story to our Respiratory Department Team at AristaCare East Falls.

Success Story: A Remarkable Recovery
On January 28, 2026, Mr. Jaug’s life took a sudden and serious turn when he was involved in a motor vehicle accident. As a restrained driver, he had taken proper safety precautions, yet the impact left him nonresponsive at the scene. He was transported to STACH, where medical teams immediately began critical interventions to stabilize his condition. In the weeks that followed, Mr. Jaug faced significant medical challenges, including respiratory failure. On February 27, 2026, he was admitted to Arista Care East Falls for continued care and rehabilitation. At that time, he required respiratory support, reflecting the severity of his condition. Despite the gravity of his injuries, Mr. Jaug demonstrated resilience and determination throughout his recovery journey. With the support of a dedicated healthcare team, he made steady progress. Day by day, his strength improved, and his dependence on respiratory support decreased. A major milestone was reached on March 23, 2026, when Mr. Jaug was successfully decannulated and no longer required supplemental oxygen. This achievement marked a significant turning point, highlighting not only his physical recovery but also his perseverance and commitment to healing.
Mr. Jaug’s journey is a powerful testament to the impact of a dedicated respiratory team, qualified nursing care, and patient determination. From a life-threatening accident and respiratory failure to breathing independently again, his story stands as an inspiring example of recovery against the odds.

I'm Happy and excited to share another wonderful, amazing successful Discharge at East Falls.!!!!! No pictures just the story.

Patient admitted to AristaCare East Falls in September 2025 s/p ICH and SAH coupled with respiratory failure. At time of admission, patient required supplemental oxygen via trach collar to meet respiratory needs. He was unable to eat or drink food by mouth and received all medication and nutrition via PEG tube. Patient was totally dependent upon staff for all activities of daily living, posture correction and repositioning, and all aspects of functional mobility. Patient made tremendous strides with all rehab disciplines and respiratory therapy throughout his stay at East Falls. Respiratory therapy was able to aggressively wean patient from trach collar to room air, and patient was able to be decannulated only 30 days after admission. Speech therapy worked to improve patient cognition and swallow function – at time of discharge patient progressed from NPO all the way to being able to eat regular diet with thin liquids without risk for aspiration. Additionally, speech therapy improved upon patient cognition, memory recall, and speech, and at time of discharge patient is able to verbally make his needs known and hold conversational speech. Throughout his work with physical therapy, he improved upon strength, balance, and independence with mobility. Patient is able to perform standing tasks with minimal assistance, perform bed mobility with supervision, and is able to tolerate being out of bed in his personal customized tilt in space wheelchair. Occupational therapy facilitated improved upper extremity strength and coordination, resulting in improved independence with ALD tasks as patient is able to perform self-feeding with independence and requires minimal assist with dressing and grooming tasks. After 6 months of high intensity rehabilitation, patient is ready for discharge. We wish him all the best in his continued rehabilitation journey at home with family!!!!

Here is another amazing success story at AristaCare East Falls. Ms. Mariam admitted to AristaCare East Falls in September of 2025 with a diagnosis of Respiratory failure and COPD exacerbation. Upon admission Mariam required 40% oxygen via trach collar to meet respiratory needs, she was NPO and unable to eat or drink food by mouth, she required Maximal level of assistance ranging to total dependence with mobility and activities of daily living. Mariam worked diligently with rehab services throughout her stay and demonstrated significant progress with all aspects of care. With respiratory therapy Mariam was able to wean down her oxygen needs to 28% trach collar, she progressed to a regular diet with thin liquids and was able to have her PEG tube removed. Occupational therapy was able to facilitate improved independence with ADLs, and at time of D/C Mariam is able to perform all bathing, dressing, and toileting tasks with modified independence. Mariam also improved with her strength, dynamic balance, and mobility while working with physical therapy. She is able to perform 300 feet ambulation using RW and ascend/descend full flight of stairs with supervision level assist. Mariam was a joy to work with and always brought a smile and a great sense of humor to each therapy session. After 5 months of hard work with therapy, Mariam is ready to safely discharge home with her independence restored and with reduced risk for falls. We are very proud of you Mariam, and we wish you all the best in your transition home!
