Job Description

Job Title: In-Home Safety Assessor

Reports To: Clinical Director, Agency Administrator, or Program Manager

Position Type: PRN, or Independent Contractor

Position Summary

The In-Home Safety Assessor evaluates residential environments to identify hazards, assess client functional abilities, and recommend modifications, equipment, and supportive services that promote independence, reduce fall risk, and enhance overall safety. Assessments may be conducted for older adults, individuals with disabilities, post-hospitalization clients, veterans, and caregivers.

Essential Duties and Responsibilities

* Conduct comprehensive in-home safety assessments for clients in private residences, apartments, assisted living settings, or family homes.
* Evaluate environmental risks including:
* Fall hazards
* Lighting deficiencies
* Trip hazards
* Bathroom safety concerns
* Fire and emergency preparedness
* Medication storage and management
* Accessibility barriers
* Kitchen safety concerns
* Assess clients’ mobility, balance, transfers, activities of daily living (ADLs), and cognitive status as applicable.
* Identify the need for adaptive equipment such as:
* Grab bars
* Raised toilet seats
* Shower chairs
* Bed rails
* Ramps
* Personal emergency response systems
* Medication reminders
* Educate clients and family members on strategies to prevent falls, injuries, and avoidable hospitalizations.
* Develop individualized safety recommendations and action plans.
* Complete detailed assessment reports and maintain documentation in accordance with agency policies.
* Coordinate referrals to community resources, home care services, therapy providers, contractors, or durable medical equipment vendors when appropriate.
* Communicate findings to physicians, case managers, family members, insurers, or care coordinators as authorized.
* Maintain confidentiality and comply with HIPAA and state regulations.

Qualifications

Preferred candidates may include:

* Registered Nurse (RN)
* Licensed Practical Nurse (LPN)
* Occupational Therapist (OT)
* Physical Therapist (PT)
* Certified Aging-in-Place Specialist (CAPS)
* Social Worker
* Certified Dementia Practitioner (CDP)
* Experienced Home Care Professional with specialized training in home safety assessments

Knowledge and Skills

* Understanding of aging-related conditions and disability accommodations
* Knowledge of fall prevention principles
* Familiarity with ADA accessibility guidelines
* Strong observational and problem-solving skills
* Excellent verbal and written communication skills
* Ability to work independently and manage scheduling
* Proficiency with electronic documentation systems

Physical Requirements

* Ability to travel to client homes.
* Ability to navigate stairs, uneven surfaces, and various home environments.
* Ability to lift or carry assessment materials and adaptive equipment samples up to 25 pounds.

Compensation Examples

* Arizona: Approximately $ 50 per assessment, depending on assessor credentials, assessment complexity, and payer source.
* Mississippi: Approximately $40 per assessment, depending on credentials and service area.

Navigating insurance programs and plans can be a tricky and confusing experience. Here is a quick comparison of Medicare and Medicare Advantage and how they match up to what long term care insurance offers.

Bottom Line
Original Medicare:
With Original Medicare you can receive treatment virtually anywhere but you have to pay additional premiums for different plans. If your premium covers services and supplies you do not have to request it. You are not covered for care outside of the U.S. unless you buy a Medigap policy
Cost:
Plans set the amounts they charge for premiums, deductibles, and services
You pay 20% of Medicare-approved amount after you have reached your deductible
Medicare Advantage:
With Medicare Advantage you must stay within the network and may require referrals to see a specialist. Out of pocket costs vary with services and if you pay a premium or not but there is a limit for part A- and B-covered services that when met will not cost you anything for the rest of the year. Plans cover the same things as Original Medicare plus more services such as vision, hearing, and drug coverage. Medicare Advantage also covers more assisted living than Original Medicare to include daily assistance in the home, respite, and transportation. Plans do not normally cover outside of the U.S. or emergency situations handled outside of the network.
Cost:

You have to pay the Part B premium. In 2019, the standard Part B premium amount is $135.50 (or higher depending on your income).
Out Of Pocket varies on what plan you pick, whether you stay in network, how much you pay for copay, and the type of health care you receive and how often.
Original Medicare
Doctor and Hospital Choice

You can go to any doctor or hospital that takes Medicare, anywhere in the U.S.
In most cases you don’t need a referral to see a specialist.
Cost
For Part B-covered services, you usually pay 20% of the Medicare-approved amount after you meet your deductible.
You pay a premium (monthly payment) for Part B. If you choose to buy prescription drug coverage (Part D), you’ll pay that premium separately.
There’s no yearly limit on what you pay out-of-pocket unless you have supplemental coverage (like a Medigap policy).
You can get supplemental coverage(like a Medigap policy) to help pay your remaining out-of-pocket costs (like your 20% coinsurance). Or, you can use coverage from a former employer or union, or Medicaid.
Coverage

Plans generally don’t cover care outside the U.S. Also, plans usually don’t cover non-emergency care you get outside of your plan’s network.
Long term care insurance
Long term care insurance is health insurance whose focus is to aid in chronic illness often attributed to aging. Services generally cover care in your home, nursing homes, assisted living facilities, and adult day care centers. This type of insurance is important for covering long term stays, on-going care, and in-home health that most insurances do not.

Medicare only covers short nursing home stays or a limited amount of home health care. Assistance with daily living is not covered by Original Medicare though some aspects are covered by Medicare Advantage.

The best way to avoid running through benefits is knowing what you have access to and comparing them to ensure you are using what you need and not what you don’t.

It’s a Process.

Oh! I can attest to this! I have had awesome people apply to work with me here at Angie‘s Angels home healthcare and through Life Strategies Consulting; however I learned some valuable lessons in passing people through while neglecting my core values and process.

Usually what happens is the work that’s necessary does not meet the standard by the person doing the work because they are accustomed to not meeting a standard.

I’ve noticed that working for a small business to some indicate that the information is not important, that the process and procedures are not important. Therefore, the job is not taking as seriously as if it were with a fortune 1000 or fortune 500 company. I feel like my agencies are just as important as any large, national agency or even international agency.

I have had many feelings hurt… both mine and someone I brought aboard . What I found out is nobody loves your baby more than you, but whoever is in and around your baby and handling your baby needs to like it enough to take care of it when you are not around.
