Malleolus Monday(?)!

Young female with trimalleolar ankle fracture subluxation that was splinted without reduction. Presented to office 3 weeks out so we went to OR next day to fix the fractures. Triple anti-glide plates! We scoped at the end to assess the syndesmosis, cartilage, and our reduction. Scope shots show the posterior fragment came together very nicely!

Arthroscopic hindfoot fusion!
Patient has increased BMI, uncontrolled diabetes, and severe peripheral neuropathy, with an open ankle fracture.

Given their risk factors for wound issues and nonunion, we planned for a primary hindfoot fusion to more reliably give the patient a stable leg and ideally avoid amputation in the future. We do the joint prep arthroscopically to keep incisions small and preserve blood supply.

1/2 - injury films
3/4/5 - arthroscopic joint prep
6 - reduction XR
7/8 - final fluoro shots

Awesome lab session with the residents, going over mid foot trauma and fusions, Jones fractures, ankle instability, and all the different ways to fuse an ankle. Trauma Tuesday!

Young man with open comminuted both bone forearm fracture. Great work with the residents last night putting it back together!

Open fractures (aka “compound fractures”), are surgical urgencies to get cleaned up and stabilized to help decrease risk for infection and other complications. They are often higher energy injuries as well.

1&2 - injury X-rays
3&4 - intra op work
5&6 - plates and screws in place
7&8 - intra op x rays
9&10 - post op films
