He almost gave up golf. β›³

A patient of mine β€” semi-retired, plays golf three times a week, Scottsdale area β€” had been dealing with low back pain for 6 months.

He'd had steroid treatments, feeling better each time, then the pain came back.

His orthopedic surgeon said he wasn't yet a surgical candidate. Suggested PT. He was skeptical β€” he'd tried a generic PT program years earlier, and it didn't stick.
But he was running out of options.

When I assessed him, the picture was clear: restricted thoracic rotation, limited hip rotation on his lead leg, and a compensatory pattern where his lumbar spine was doing the work that his mid-back and hips should have been handling.

None of those issues had been addressed by his previous treatment.

Nine weeks later, he's back on the golf course. Within 10 weeks, he’s playing 18 holes 3x per week, and hitting more than his normal distance. He is playing β€œamazingly and my back feels better”- his words.

But here's the thing: "a little tight" rarely stays "a little tight."

The compensation patterns your body makes to protect a tight or painful

Golf back pain is the most common reason adults over 55 stop playing.

But in most cases, it's not a golf problem.

It's a movement problem that is expressing itself in your golf game.

Here's what I mean:

The golf swing requires about 90 degrees of thoracic (mid-back) rotation. Most adults over 55 have significantly restricted thoracic mobility β€” often without knowing it β€” because of decades of sitting, compensation patterns from old injuries, or simply age-related stiffness.

When the thoracic spine can't rotate adequately, something else has to. Almost always, the lumbar spine (low back) takes over.

The lumbar spine is not designed for rotation. It's designed for stability. When it's forced to rotate repeatedly under load β€” which is exactly what the golf swing demands β€” the result is predictable: pain, disc stress, muscle strain, and inflammation.

Here's what most golfers do next: they get a steroid injection. The inflammation quiets down. They feel better for 6–8 weeks. Then the pattern resumes and the pain returns.

Here's what actually fixes it: restoring thoracic mobility so the lumbar spine can return to its stabilizing role. Combined with hip mobility work β€” because the downswing requires significant hip extension and internal rotation that most golfers have lost β€” and core activation patterns that protect the spine under load.

This is exactly what I address with golfers in my practice.

I've treated pickleball injuries over the years. And I keep seeing the same mistakes that turn a minor irritation into a 3-month setback.

Here are the top 3 mistakes active pickleball players make that lead to injury:

❌ Mistake 1: Playing through pain without addressing the cause

Pain is a signal, not a challenge to overcome. Playing through knee or shoulder pain changes your movement patterns in ways that create NEW injuries while the original one gets worse.

❌ Mistake 2: Treating symptoms instead of causes

Icing your knee helps it feel better today. It doesn't fix the hip weakness pattern that's loading your knee wrong on every lateral step. You'll be icing the same knee in six months.

❌ Mistake 3: Using rest as the primary treatment strategy

Rest helps. But when you return to play after two weeks off with the same movement pattern, you're right back where you started β€” usually within a session or two.
