Responsibilities
• Complete the intake process from initial referral contact to homecare admission
• Provide direct oversight of patient education and preparation for homecare, plan of care initiation, and coordination of care with multiple service providers, this includes coordinating and delivering clinical information to the patient's clinical team
• Utilize clinical knowledge to triage incoming referrals to determine the necessity of timeline initiation of care and needs
• Communicate directly with patients and families to ensure acceptance of services and coordinate the insurance verification and authorization processes
• Document pertinent information regarding the patient's demographics, medical condition, medications and treatments, and service being requested
• Communicate homecare criteria to referral sources and serve as a resource for homecare criteria and alternatives to internal and external customers
• Ensure face-to-face visit has been arranged if needed
• Establish and maintain positive working relationships with current and potential referral sources and clinical team
• Ensure compliance with all state and federal legal and regulatory requirements

• Graduate of an accredited school of professional nursing required
• Minimum of 2 years of nursing experience (home health preferred)
Licensure, Certifications, and Clearances:

• Oasis Knowledge, developing patient POC (preferred)
• Automotive Insurance/ Registration

Job Description
Western Pennsylvania Home Health Association is looking for a full-time Clinical Intake Coordinator RN/LPN to help support our team of front line heroes!

Responsibilities:

The Clinical Intake Coordinator performs a variety of administrative duties in WPHHA's office, including, but not limited to:

Verify necessary information and records in the medical record and computerized scheduling system.
Coordinating patient referrals.
Scheduling field clinicians and provide administrative support for clinicians.
Coordinate employee schedules and caseload.
Review and track clinical intake information.
Work closely with the Director of Nursing to ensure ongoing compliance.
Coordinate patient supplies.
Relay necessary messages to key clinical staff and providers.
Input new patient information including coordination of patient care.
Assist DON with QAPI, clinical engagement, patient, and provider resolution.
Maintain strict confidentiality related to medical records and other data.
Qualifications and Skills

Proficient in Microsoft Office suite, Excel, and adobe
Must be able to use good and sound judgment.
Must be able to document accurately and appropriately.
Demonstrates patience, flexibility, and a cooperative attitude.

Job Description

WPHHA is looking for a competent Nursing Clinical Director to organize and oversee our team and programs. You will be responsible for supervising and directing everything from employees and budgets to technology and operations. As a Nursing Clinical Director, you must possess excellent management and mentoring skills. You must be an outstanding communicator able to resolve conflicts and issues. If you have a deep respect for policies and ethics and can inspire that in others, you are ideal for the role. The goal is to ensure the smooth running of the clinical department aiming to maximum performance.

Essential Duties and Responsibilities

• Responsible for both clinical and administrative functions related to Home Health
• Provides leadership, guidance, and direction in achieving outcomes and operational performance consistent with the mission, vision, and goals
• Protects the agency CMS star rating through performance consistent with mandated policies and regulations, including upholding agency accreditation.
• Assumes responsibility for the assessment, planning, and implementation of a clinically efficient and effective system of care and service within the continuum of care
• This position will also serve in a vital accreditation role in assuring compliance with regulatory standards, requirements, and guidelines
• Attend and lead required orientations, meetings, and in-services
• Demonstrate a commitment to flexible work scheduling when necessary to ensure patient care
• Perform Quality Assurance review on clinician documentation and performance included but not limited to the following:
• Review Plan of Care notes from clinicians for completeness, accuracy, and consistency.
• Review number of visits relative to patient diagnosis
• Review Start of Care assessment for completeness, accuracy, and consistency
• Monitor clinician activities including but not limited to the following:
• Therapy use (LUPA episodes)
• Number of visits made
• Readmission rates
• Home Health Compare performance at the clinician level

• Supply use
• Managing PTO coverage
• Missed visits
• Timeliness of initiation
• Overtime
• Conduct orientations for new clinicians as well as promote clinician skill development
• Directs implementation of approved work policies and procedures that reflect elements essential to rendering high-quality care.
• Conducts clinician education when improvement is required
• Provides leadership in promoting and maintaining standards for providing excellent quality care by all members of the patient care team

Education and Experience Minimum Required Education:

Experience in clinical supervision and program management
Certification in specialty (ie. Oasis)
2 years of leadership in a similar industry preferred.
Solid understanding of budgeting, resourcing, and performance evaluation procedures
Excellent knowledge of standards (e.g. CMS) and regulations for the clinical home health field staff
Proficient in MS Office and computer systems (e.g. patient management software)
Excellent communication and people skills
Exceptional organizational and leadership skills
Aptitude in resolving issues and conflicts
RN/BSN Required
