You started reaching for the railing.

You're not sure exactly when it became automatic. Going up is fine... it's coming down that you brace for now. One step at a time. Maybe a little sideways.

Most people chalk this up to age and reach for ibuprofen. But this specific pattern — pain down, not up — isn't random. Your knee is naming something pretty specific, and it's usually one of the earliest signals that something's changing in there.

The good news: caught early, this is a very different conversation than the one that happens two years from now.

"Just give it time."

That advice has an expiration date most people don't know about.

The tissue you have at week 6 isn't the tissue you have at month 6. And the conversation about what to do gets very different depending on which one you're working with.

05/07/2026

"It's manageable."

That's the sentence most people use to put the decision off another six months.

Then another.

The body doesn't pause while you think about it. Worth understanding what's actually happening in there while you wait.

Save this. Send it to someone who's been "dealing with it" for too long.

After two knee surgeries and two shoulder surgeries, she had hit the wall most active people eventually hit.

The gym hurt. Walking hurt. Her doctors told her a knee replacement was the next step. She wasn't ready to accept that.

So when a flyer came in the mail — sent by the school district she'd taught at for 36 years — mentioning Regenexx, she made the call. Not because she was sold. Because she figured it couldn't hurt to ask.

Here's what her life looks like a few weeks out:

Walking on uneven sand at the beach. No pain.
Walking through the park on rough ground with her husband. No pain.
Spring landscaping, mulching, raking — the kind of work that used to wreck her shoulder. No pain.

Her exact words: "I feel great. Absolutely great."

We share stories like this because the people who need to hear them are usually the ones who've already been told their only option is more surgery. They're the ones quietly cutting back on the things they love because pushing through hurts too much.

If that's you or someone you love, you have more options than you've been told.

Every patient is different. Every joint is different. But the conversation is worth having before you say yes to the next surgery.

You've been on the "treatment treadmill" longer than you wanted to be (and gotten nowhere).

Pills. PT. A shot. Another shot. And now someone's telling you it's time to schedule the surgery.

If something inside you is hesitating, that's worth listening to.

Watch this before you decide.

"Bone on bone."

Two words that have sent millions of people straight to surgery.

But here's what most people don't know: what the X-ray shows and what's actually causing your pain aren't always the same thing.

Studies have shown the same finding over and over — some people with severe joint space narrowing barely feel it. Others with mild narrowing can hardly walk.

The image doesn't tell you what's generating the pain.

It could be the cartilage. Or it could be inflamed soft tissue. Unstable ligaments. Joint inflammation. Things that have non-surgical options.

Before you accept "bone on bone" as a surgery mandate, ask one question:

"What specifically is causing my pain right now — and have we tried every non-surgical option for that exact structure?"

That one question can change the entire conversation with your doctor.

You did the PT. You showed up. The pain still came back.

It's not because you didn't try hard enough.

Physical therapy is great at building strength and improving how you move. But here's what PT can't do: repair the actual damaged tissue.

If you have a partially torn ligament, a tendon that healed wrong, or a joint that stays inflamed — strengthening the muscles around it won't fix what's underneath.

That's why the pain calms down for a few months… then comes back the moment you stop the exercises.

It's not a PT failure. PT is doing its job.

It just means there's something structural underneath that needs a closer look.

If your pain keeps returning, that's the signal. It's worth an honest conversation about what's actually going on in there.

Save this for the next time someone tells you to "just do more PT."

A patient sent us this handwritten note and we had to share it.

51 years old. Fastest woman on her squad (except for one 28-year-old, but who's counting).

This woman did more than just maintain her fitness after receiving regenerative treatment. She set a personal record.

Age is not the ceiling people believe it is. Our goal is always to keep you doing what you love, better than before.

Pickleball was supposed to be the fun one. The sport that didn't wreck your body.

So why does your knee have a different opinion every time you step on the court?

Here's what most players figure their options are: play through it and pay for it later, or get surgery and miss months of everything — the games, the group, the Tuesday mornings that are honestly the best part of your week.

But there's a third option most people never hear about.

Regenexx is a non-surgical regenerative treatment that uses your own cells — concentrated platelets or bone marrow-derived stem cells — placed directly at the source of the problem using precise image guidance.

No guesswork. No hardware. No months on the sideline watching your group chat light up without you.

Our physicians at Regenexx Pittsburgh have worked with plenty of pickleball players dealing with the usual suspects — knee pain, shoulder issues, back problems. This sport is more demanding than it looks, and your body knows it.

Many patients see improvement in weeks, not months. Many get back on the court faster than they expected. Some avoid surgery entirely.

Not everyone is a candidate, and we'll tell you that straight up. But here's what we know:

Before you give up the court, the crew, and the thing that gets you out of the house and moving — it's worth finding out what's possible.

Pickleball isn't just exercise. It's your people. And that's worth protecting.

If you're in Western Pennsylvania and joint pain is threatening your game, tap the link in our bio and fill out a quick candidate form. We'll review your case honestly — and if there's a path back to the court, we'll show you what it looks like.

You're not training for anything. You just want to get through Tuesday without your knee deciding otherwise.

That's not a small goal. That's the whole point.

And yet somewhere along the way, you got stuck in a loop — medication, ice, "take it easy," repeat.

Nobody's actually addressing why you're in pain. They're just helping you tolerate it.

That's the cycle we help people break.

Regenexx is a non-surgical regenerative treatment that uses your own cells — concentrated platelets or bone marrow-derived stem cells — placed precisely where the damage is using advanced imaging guidance.

Not a blind injection and a "let's see what happens."

Not hardware bolted into your body.

Not months on a couch recovering from an operation you weren't sure you needed.

It's real medical care, performed by physicians, right here in Pittsburgh.

We've worked with many patients who came in saying the same thing: "I'm not looking for a miracle. I just want to function." Some saw improvement within weeks. Others took longer. Some avoided surgery they thought was inevitable.

And some weren't candidates at all — and we told them that upfront.

Here's what we won't do: promise you results we can't predict or push you toward something that doesn't make sense for your situation.

Here's what we will do: look at your case honestly and tell you whether this approach could help address what's actually causing your pain — not just cover it up again.

If you're tired of building your entire day around what hurts, tap the link in our bio and fill out a quick candidate form. We'll take it from there.
