Sometimes what looks like a surgical issue is really just back to mild baseline anatomical asymmetry.

In this case, the orbicularis muscle that encircles the upper and lower eyelids is stronger on the left side. This means that when our beautiful patient smiles, she sees the left eye look “smaller” due to the overriding muscle tone.

To create more symmetry, we carefully injected along her left lower eyelid margin and just within a few days, our patient was smiling without feeling self conscious and with more symmetric eye opening.

It’s no secret that all physicians are therapists in one shape or form, whether simply by listening, examining, or operating.

Sure, plastic surgery can offer a great self esteem boost and we are all for that.

BUT if you are hoping that plastic surgery will resolve deep seated personal issues, then this warrants a pause for a much longer and introspective conversation.

Do not hinge the success of your marriage or other personal and professional achievements on your surgical outcome - we can’t fix marriages, sorry. (Btw this scenario 100% happened to us a few times)

…is the one you’ll commit to using EVERY DAY.

There are several effective eye creams that will properly hydrate the thinnest skin on the body, i.e. the eyelids.

Whether you spend $10 or $250, just be consistent and you’ll appreciate healthier looking eyelid skin.

And let’s get this straight: do NOT expect surgical results from using an eye cream. That’s not how it works. If you need to reduce eye bags or heavy skin, no eye cream on the planet will solve your problems. Turn to an oculofacial plastic surgeon for best practices 🎤

🚫 NO fat repositioning / grafting
🚫 NO skin excision / pinch / incision

Just straightforward judicious fat excision using a behind-the-lid technique leaving behind no visible scar.

Hot take - can cause more problems than it solves. Remember that your face is a dynamic continuum - skin and soft tissues are losing elasticity and thickness, while your facial skeleton is flattening out at its prominences.

Where fat is repositioned to now likely isn’t the right position in only a few years to come. And when fat doesn’t settle in nicely (happens more commonly than you think), you get lumps under your eyelid skin and the only solution is surgical.

This is why we don’t do fat repositioning or grafting into the lower eyelids. Also remember that it is NORMAL to have a concavity from the side of your nose extending into the inner corner of your lower eyelid.

One of the most common questions we get asked is, how do we get rid of our eye bags?

Now - there are some bags which are pr*****ed fat pads that nothing but surgery will resolve.

We’re talking about the eyelid swelling that’s very common when you awake in the morning. This is a phenomenon called “dependent edema” and occurs because we are laying flat for several hours so the fluid pools around our eyes with nowhere to go. Within a short period of time after awakening, the fluid gravitationally redistributes and some of that puffiness goes away on its own.

Add to this high sodium intake, environmental allergies, sleeping on your stomach - and it’s possible to have almost a 10% fluctuation in your lower eyelid appearance from day to day.

One simple fix is sleeping with a slight incline - like staggering two pillows for a ~20ish degree tilt. (Of course make sure your neck is comfortable at all times.) Just this one modification alone will allow you to wake up in the mornings with minimal to zero eye puffiness.

A second bonus hack? Use a small (jade) eye roller and gently sweep from inner corner of lower eyelids adjacent to your nose all the way out towards your ear. Help steer the excess fluid out towards your lymphatics so they can do their job and regulate.
