We always appreciate the opportunity to connect with fellow healthcare professionals who share our commitment to supporting seniors and their families.

Recently, our Director of Client Care, Azizi Traylor, had the pleasure of presenting to the wonderful team at Alameda Care Hospice in Walnut Creek.
Together, they discussed how hospice and in-home care can work hand in hand to provide additional support for patients and families during life's most challenging transitions. From helping families understand available resources to identifying when extra care may be beneficial, these conversations are an important part of ensuring every family feels supported.

Thank you to the social workers, chaplains, and the entire team at Alameda Care Hospice for the warm welcome, thoughtful questions, and meaningful discussion. We truly value opportunities like these to collaborate with partners who are just as passionate about compassionate, person-centered care as we are.
