Grateful to have been part of Sutter’s inaugural ENT Conference in the San Francisco Bay Area. Such a great opportunity to connect with ENT colleagues from across the West Coast, share ideas, and get a glimpse into Dr. Gaelen Stanford-Moore’s practice.

It is especially meaningful to see former trainees building their own practices and helping shape the next generation of facial plastic and reconstructive surgery. So proud of Gaelen and all she is doing.

And always a treat to learn from Michael Fritz—his free flap expertise and nasal reconstruction results never cease to impress me.

A wonderful few days of learning, reconnecting, and celebrating our specialty.

Before and After | Straightening Rhinoplasty.

Why are crooked noses so challenging to straighten? Because nothing in the nose exists in isolation. The nasal bones, septum, cartilage, tip, and nostrils all influence one another. A small change in one area can affect the shape—and function—of the areas around it.
In this young man, the bridge and tip were significantly deviated, and his nasal obstruction was part of the problem as well. Our goal was to bring his nose closer to the midline, improve his breathing, and preserve a natural appearance that still looked like him.
When starting with a significantly crooked nose, I counsel patients that a realistic expectation is meaningful improvement. Subtle contour differences can still appear when looking very closely or under different lighting and shadows.

I’m so pleased with how naturally his nose fits his face—and even happier that he is breathing better.
Thank you for trusting me with your care. 🤍

Many patients don’t realize that having a fantastic plastic surgeon is only part of the equation for achieving optimal results. The one doing the real heavy lifting following a plastic surgery procedure is you.

Your plastic surgeon can take you a long way, but after surgery, it’s time to put in the work to get yourself across the finish line with the right foods and drinks to help your body heal.

Before and After | Revision Rhinoplasty.

This patient has very thin nasal skin. Over time, the skin had contracted tightly around the underlying tip structure, creating visible contour irregularities and a concavity just above the tip.
For her revision, our goal was not to dramatically change her nose. Instead, we worked to soften the tip, restore a smoother transition from the bridge to the tip, and augment the nasal dorsum to create more balanced proportions.
With thin skin, even very small differences in the underlying cartilage can become visible, so creating smooth, well-supported contours is especially important.

I love how natural she looks—still completely herself, with a softer and more harmonious nasal contour.

Before and After | Rhinoplasty.

One of my favorite things about Rhinoplasty is that there is no single operation that works for every nose.
This beautiful patient and fellow healthcare professional came to me with difficulty breathing, deviation of both her nasal bridge and nostrils, and a very delicate nasal framework. Her skin was thin and her native cartilages were also quite delicate.
Our goal was not to give her a different nose. It was to help her breathe, straighten what was deviated, and build support while preserving the features that make her look like herself.
That required a graft designed specifically for her anatomy. I used a hybrid of a lower lateral crural strut and an alar rim graft, extending support farther into the ala where her native cartilage did not naturally reach. This helped strengthen and stabilize the nostril while improving its shape.
These are the details I love about Rhinoplasty: understanding the anatomy in front of you and adapting the operation to that individual nose rather than trying to make every nose fit the same technique.

She is healing beautifully, still with more settling ahead, and I am so pleased for her. 🤍

Reflecting back on a fantastic Portland Rhinoplasty Course at the end of July!
We had an incredible group of faculty and practicing surgeons come together for a weekend of learning, discussion, and hands-on education. This year, we expanded the course to include two cadaveric dissection sessions, which gave us even more opportunity to translate concepts into surgical technique.

One of my favorite parts of the course is hearing how different surgeons approach rhinoplasty—the techniques they rely on, the philosophies that guide their decision-making, and how and why they are incorporating preservation rhinoplasty into their practices.

So grateful to our outstanding faculty and to everyone who traveled to Portland and contributed to such thoughtful discussions.

Already looking forward to next summer!
Mark your calendars: — Aging Face is up next! Last weekend of July 2027. Looking forward to another opportunity to learn together in Portland.

Then mark your calendars for the last weekend of July 2028, when we’ll do it all again for the next Portland Rhinoplasty Course.

Before and After | Rhinoplasty | Dorsal Hump Reduction & Tip Refinement.

This patient wanted to improve the appearance of her profile while maintaining a natural look.
She had a deep radix, a dorsal hump, and a fuller nasal tip. Rather than creating an overly reduced nose, our goal was to preserve an elegant, elongated profile that remained in harmony with her facial features.

Her Rhinoplasty included:
· Refinement of the nasal bridge with preservation of a graceful dorsal line.
· Tip refinement while maintaining adequate support.
· Correction of the deep radix to create a smoother transition from the forehead to the nose.
· Preservation of proportions appropriate for her moderately thick skin.

The result is a nose that appears softer, more balanced, and feminine while still looking completely natural and fitting her face.

Grateful to have been invited to speak at the Pacific Coast Oto-Ophthalmological Society ( ) meeting in Hawaii. It was a privilege to discuss two topics I care deeply about—Rhinoplasty and Facial Paralysis—with such an engaged and collaborative audience.

If you’re looking for a meeting with outstanding education, genuine collegiality, and a destination the whole family can enjoy, I highly recommend PCOOS.

One of the things that makes this meeting so unique is the partnership between oculoplastic and facial plastic surgeons. Working closely with Dr. John Ng at Casey Eye has shown me firsthand how much our patients benefit when our specialties learn from one another.

It was also an honor to follow Dr. Carol Bradford’s keynote. She was the first female Chair of Otolaryngology I ever met and has inspired many of us through her leadership. Congratulations on your new role as Dean of the Medical School at the University of Minnesota!

Thank you to the organizers for creating such a wonderful meeting. Its smaller size fosters meaningful conversations, new collaborations, and lasting friendships—all in an incredible setting. As a bonus, I was lucky enough to swim with a honu (Hawaiian green sea turtle), a memorable reminder of Hawaii’s beauty.

Before and After | Crooked Nostrils.

Did you know your nostrils can provide clues about what’s happening inside your nose? 👃
Sometimes, asymmetry of the nostrils isn’t just a cosmetic concern. A crooked or off-center nostril can be a sign of a deviation of the front (caudal) septum—the part of the septum that helps support the tip of the nose.

When this type of septal deviation also causes nasal obstruction, surgery can often:
✔️ Improve breathing
✔️ Straighten the support of the nose
✔️ Improve nostril symmetry as a natural consequence

Every nose is unique, and not all nostril asymmetry is caused by a septal deviation. A careful examination is needed to determine the underlying cause and whether surgery would be beneficial.

These photos show one example of improvement in nostril symmetry after correcting a caudal septal deviation.

Honored to contribute a chapter on Skin and Soft Tissue Management in Rhinoplasty in this outstanding Rhinoplasty text edited by and Edward H Farrior!

When discussing Rhinoplasty, we often focus on cartilage and structural support, but the skin-soft tissue envelope plays a critical role in the final aesthetic result—especially in the nasal tip. Understanding how skin thickness, soft tissue characteristics, healing, and surgical technique interact is essential to achieving natural, long-lasting outcomes.

Grateful to have collaborated on this chapter with Kayva Crawford, MD | San Diego Facial Plastic Surgeon and Peter Eskander, and thankful to the editors for the opportunity to contribute to this incredible resource.
