Being a good Director of Nursing doesn’t automatically mean you know how to handle every situation that lands on your desk.

Staffing issues. Survey preparation. Difficult families. Clinical concerns. Documentation. Managing your team while still answering to administration and ownership.

It’s a lot, especially for someone who is new to the role.

Some of the best DONs we’ve worked with didn’t need someone to come in and take over. They needed someone experienced in their corner who they could call, ask the question they weren’t sure about and work through the next step with.

Sometimes that support is what turns a good nurse into a really strong leader.

You can have the right policy sitting in a binder and still have a compliance problem.

Because the real question isn’t only, “Do we have a policy for this?”

It’s whether your team knows it, understands it and is actually following it in their day-to-day work.

As we expand our consulting services to Rhode Island home care agencies, this is one of the areas we’ll be spending a lot of time on. We can help agencies review what’s currently in place, identify where there may be gaps and, most importantly, make sure policies and procedures make sense for the way the agency actually operates.

Compliance shouldn’t exist only on paper.

One of the things we say often at Innovations is that you don’t have to wait until there’s a crisis to bring in outside support.

Sometimes things aren’t necessarily “wrong.” Your leadership team is just stretched thin. Census isn’t where you want it. A new DON needs support. There are survey concerns that keep getting pushed to next week. Or everyone is so busy getting through the day that no one has the time to step back and look at the bigger picture.

Those are often the moments when an outside set of eyes can make the biggest difference.

Our work isn’t always about coming in to fix a facility in crisis. Sometimes it’s about helping a good team get ahead of what’s coming next.

We’re expanding the way we support healthcare providers in Rhode Island.

Over the past six years, Innovations Healthcare has worked alongside nursing homes and long-term care facilities through regulatory challenges, operational changes, staffing needs and the day-to-day realities of providing great care.

Along the way, we recognized another area of Rhode Island healthcare where that same kind of hands-on support is needed: home care.

Today, we’re excited to officially introduce Home Care Agency Consulting at Innovations Healthcare.

We’ll be working with Rhode Island home care agencies on regulatory compliance, policies and procedures, QAPI, staff education and competencies, operational planning, governing body support and navigating requirements with RIDOH.

This is a new area of service for Innovations, but the way we work isn’t changing. We’ll continue to be the team that gets in there with you, figures out what needs to happen and helps you actually get it done.

We’re looking forward to bringing that approach to Rhode Island’s home care community.

Strong facilities don't suddenly become survey-ready when the Department of Health walks through the door.

They're built in the everyday moments. The morning huddles. The documentation that's done correctly the first time. The leaders who coach instead of react. The small issues that get addressed before they become big ones.

Survey results are usually a reflection of the systems you've built long before survey week begins.

If you're constantly feeling like you're preparing for the next survey, it may be time to stop preparing and start building stronger processes that work every day.

That's where we come in.

One of the biggest misconceptions in long-term care is that the strongest facilities are led by extraordinary people.

The truth is, even great leaders take vacations, get sick, retire, and move on.

What separates strong organizations isn't having one exceptional person who holds everything together. It's having systems that allow good people to succeed, even when things don't go according to plan.

That's why sustainable operations matter so much. Good systems create consistency for staff, stability for residents, and confidence for leadership.

That's one of the biggest differences between putting out fires and building a facility that's prepared for whatever comes next.

Every facility seems to have that one position that's consistently difficult to fill.

For some, it's an experienced MDS Coordinator.

For others, it's a Director of Nursing, Nurse Managers, CNAs, or Infection Preventionists.

Recruitment challenges vary from market to market, but they affect every aspect of operations.

We're curious...

What role has been the hardest to recruit for in your organization this year?

Running a nursing home means making hundreds of decisions every single day.

Some are routine.

Others have consequences that affect your residents, your staff, your survey outcomes, and your financial performance.

The challenge isn't that leaders don't know what to do.

It's that they're expected to do everything at once.

That's why the strongest organizations don't try to carry every challenge alone.

They bring in experienced partners who can provide extra capacity, specialized expertise, and an objective perspective, so leadership can focus on what matters most: delivering exceptional care.

Great leadership isn't about having all the answers.

It's about knowing when to bring the right people to the table.

A Plan of Correction is not the finish line. It's the starting point.

Many facilities successfully submit a Plan of Correction, but implementation is where the real work begins.

The challenge isn't creating the plan.

The challenge is:
• Educating staff
• Monitoring compliance
• Sustaining improvements
• Demonstrating ongoing effectiveness

Long-term success comes from creating systems that support lasting change—not temporary fixes.

That's where strong leadership and accountability make the difference.

How long do you wait before bringing in outside support?

Many facilities wait until they're facing a survey, a leadership vacancy, or a compliance concern before seeking help.

But the most successful organizations often do the opposite.

They bring in support early to strengthen systems, train leaders, and identify risks before they become costly problems. Whether it's MDS support, infection prevention, social services, or operational consulting, proactive action almost always costs less than crisis management.

At Innovations Healthcare, we've seen firsthand how early intervention can make all the difference.

What areas of your operation are you strengthening today to prevent challenges tomorrow?
