Why are so many young women being diagnosed with POTS?

I’m testing for 10 mediators, but mast cells can make 1,200.

When the labs come back negative and the patient is still suffering — you don't dismiss them. You treat them.

Step one is always figuring out why they're reacting. And very often it's hiding in plain sight — chronic Lyme, Bartonella, Babesia, Epstein-Barr. Infections patients were told were resolved years ago. They weren't.

Anxiety in MCAS is not a mental illness.

It's a physiological response to dysfunctional mast cells in the nervous system.

Mast cells line the neurons. They're abundant in the brain. Every time they sense danger they fire, and the nerves short circuit. Panic. Racing heart. Impending doom.

These patients aren't anxious, their mast cells are. Treat the MCAS and very often the anxiety resolves dramatically.

PCOS and endometriosis aren't two separate problems. They're on the same spectrum.

At the root of both? Dysfunctional mast cells.

In PCOS they're driving insulin resistance and inflammation. In endometriosis, fibrosis and immune dysfunction. So many women have both, and never get told why.

When we look at mast cells as the common thread, we finally have a chance at real answers.

Mast cells have GLP-1 receptors on their surface.

That means these drugs are literally binding directly to the mast cell, signaling it to calm down and stop releasing.

No more flood of cytokines and inflammatory mediators. Just a body that can finally start to heal.
This is direct mast cell stabilization. And it's changing how I treat my patients.

A patient came to me drowning in symptoms. Mast cell. Lyme. Long COVID. Completely reactive.
We did plasmapheresis (therapeutic plasma exchange).

The process removes 2-3 liters of plasma, pulling out the cytokines, inflammatory mediators, mycotoxins, biofilms, all of it. Replaced with albumin.

One month later? Her mast cell symptoms were almost nothing.

This is one of my favorite tools in practice right now. šŸ¤

A leg swelling and turning dark purple. An ER visit. And they sent her home with nothing.

No steroid shot. No IV Benadryl. No IV fluids. Nothing.

Just "see an allergist."

This is a patient clearly showing signs of a serious immune reaction, and the system completely failed her. There are plenty of tools available in an ER to at least get a patient comfortable. The unwillingness to think, to try, to help, it's unacceptable.

Patients deserve better than this.

Endometriosis is a metabolic AND immune problem.

Mast cells are abundant in the peritoneum, and women with endometriosis show a GLP-1 deficiency in that exact area. Are GLP-1s working because they're replacing what's missing? Or because they're binding to mast cells and stopping the inflammation directly?

Either way, this changes how we should be treating it.

No gut symptoms doesn't mean no gut problems.

I see it constantly, patients convinced their issues are purely neurological. We run a stool test. Problems everywhere.

70% of the immune system lives in the gut. Mast cells line the entire GI tract. If you have a mast cell condition, the gut is involved, whether you feel it or not.
