Odysseus did not trust his own willpower, and he was right not to. He made the plan early, while he was still clear headed, and gave his crew instructions for the version of himself who would beg to be released.

Treatment plans work the same way. You decide in advance what happens on the hard days: which appointment is already on the calendar, which medication routine holds, who you call first.

A psychiatrist’s favorite question: what did you do this week that you enjoyed?

If the answer will not come, even while taking your medication every day, the treatment plan is not finished. People with chronic depression often acclimate to feeling depressed, so we measure progress objectively and aim higher than “better than before.” The goal is a full life: real energy, real enjoyment, responsibilities met.

“My antidepressant is not working.” R. Dewayne Book, MD hears this every week. Before changing anything, he checks three things: whether the trial was long enough, whether “a little better” got mistaken for the goal, and whether medication was ever meant to do the job alone.

An adequate trial of most antidepressants is two months at an adequate dose. Feeling a little better is not the goal. Remission is. If this sounds like you, bring it to your prescriber.

Meet Dan Kline, MSW, LCSW-A, Clinical Therapist at AIM.

He facilitates our men's group at the Chapel Hill office, every other Monday from 6:30 to 8:00 pm. Men from their late twenties to their sixties meet to practice what he calls emotional leadership: getting comfortable with vulnerability, without judgment and without performing.
