This is the one question you should always ask before starting Invisalign: will any of my teeth need to be filed down to make room?

It’s called interproximal reduction, and it’s a part of some Invisalign plans. It’s not filing a tooth down to a nub. It’s removing a tiny sliver of enamel, thinner than a business card, from the side of a tooth where there’s already extra space. It doesn’t hurt and it doesn’t weaken the tooth.

Here’s why I bring it up before you ever sit in my chair: not knowing this ahead of time is one of the most common reasons people feel blindsided partway through treatment. You deserve the full plan before you say yes to it, not a surprise halfway in.

Ask your provider directly: how many teeth need this, and why. If they can’t answer clearly, that tells you something too

Length, width, how much the two front teeth lead, where the edges catch light, how it all sits against his lip line / the shape of his face. Get one wrong and the eye finds it immediately, even if you can't say why.

Nobody tells you perimenopause affects your mouth too.

I hear it from patients constantly. Dry mouth that wasn't there before, gums that suddenly bleed when they didn't used to, a weird metallic taste that comes and goes. It's not in your head. Estrogen plays a real role in your gums, your saliva, and even the bone that holds your teeth in place, so when your hormones shift, your mouth often feels it first.

The good news: most of this is manageable once we know to look for it. If you're noticing changes, don't wait for your next routine cleaning to mention it.
